The Opportunity
- Deliver high-quality physiotherapy in people's own homes or care homes
- Work with a diverse and rewarding caseload (older adults, post-operative rehab, neurological conditions, reduced mobility, post-hospital deconditioning)

You'll be working across:
- Patients' homes
- Residential & nursing homes
- Community settings
While our service covers Winchester, Southampton, Fareham, Portsmouth and surrounding areas, your individual caseload will be weighted towards your chosen area to keep travel manageable and your day focused.
What You'll Be Doing
- Assessing and treating patients in their own environments
- Creating clear, structured rehab plans that patients understand and follow
- Supporting mobility, strength, independence, and confidence
- Working with families, carers, and other healthcare professionals
- Liaising with GPs, care teams, and discharge services where needed
- Managing your own diary and travel efficiently
- Maintaining accurate, timely and professional clinical records
Areas of Focus
- Elderly and frailty rehabilitation — falls prevention, mobility, strength and balance
- Post-operative rehabilitation — including joint replacements and fracture recovery
- Neurological physiotherapy — including stroke, Parkinson's, MS and other long-term conditions
Who This Role Is Perfect For
You're a physiotherapist who:
- Enjoys working autonomously but as part of a supportive team
- Wants to make a real difference in patients' day-to-day lives
- Is confident managing a varied caseload
- Has an interest in, or experience of, elderly, post-operative and neurological rehabilitation
Essential Requirements
- BSc / MSc in Physiotherapy
- HCPC Registered
- CSP Member
- Full UK driving licence
- Access to your own vehicle
Desirable Experience
- Community, domiciliary or care home experience
- Supporting patients transitioning from hospital to home
- Liaising with discharge teams / GPs
- Falls prevention / elderly rehab
- Neurological rehabilitation experience

Please note: This role has a number of non-negotiable requirements. Applicants must hold a full UK driving licence and have access to their own vehicle for work use, as this is a community-based position. Current HCPC registration is required. Candidates must also have the right to work in the UK without the need for sponsorship now or in the future, as we are unable to offer visa sponsorship or relocation support for this role.
