We are looking for a Care Coordinator to support and mentor a passionate team of Care & Support Workers
Key tasks include:
· Being the main point of contact for clients and Care & Support Workers and ensure all phone calls are answered in a professional manner.
· Responsible for allocating Care & Support Workers with the right skills and experience to deliver the best quality care and support to our clients’ individual needs.
· Organising Care & Support Worker rotas to minimise changes to the service and travel between clients’ homes.
· To ensure that all clients and team members information is updated on Mobizio/Access people planner.
· Provide exceptional customer service to our Care & Support Workers, clients, and other professionals.
· Being part of the on-call team - Rota basis.
· Covering of care calls when required.
· Various ad hoc duties as and when the branch requires.
· If you work outside of office hours you are paid at an hourly rate of £13 per hour on a weekday, £14 on a weekend (These rise to £14.00 and £14.50 respectively after a 3-month probation period).
Hours
· Full-Time Hours: 8:30am-5:00pm.
· On-call.
- Paid as overtime if covering care calls outside of office hours..
Candidate requirements:
· Previous Care Coordination or similar experience in a busy office environment (Preferred).
· Hold a full driving license and have access to your own car.
· Previous experience in a social care environment as you may be assisting Clients and carers in sensitive and emergency situations (1 year or more of domiciliary/home care experience is vital).
· Excellent team skills and an outgoing and vibrant personality.

- Salary is competitive at market rates, based on experience and a discussion with the branch manager.
Pay: £26,000.00-£29,000.00 per year
