The Role
As a Care Coordinator, you will play a vital role in the day-to-day operation of our domiciliary care service. Working closely with the Registered Manager, you will ensure that care packages are delivered safely, efficiently, and in line with each individual's assessed needs and preferences.
Key Responsibilities
- Coordinate and schedule care visits efficiently using electronic rostering systems.
- Ensure continuity of care by matching care professionals with clients based on their skills, experience, and preferences.
- Monitor daily service delivery and respond promptly to changes, emergencies, or staffing issues.
- Liaise professionally with clients, families, local authorities, NHS professionals, and other stakeholders.
- Support the assessment, review, and implementation of person-centred care plans.
- Maintain accurate electronic records and documentation.
- Assist with care staff recruitment, onboarding, induction, and competency monitoring.
- Support supervision, spot checks, and performance monitoring of care staff.
- Participate in the on-call rota and provide operational support outside normal working hours when required.
- Ensure care delivery complies with relevant legislation, safeguarding responsibilities, infection prevention requirements, and company policies.
- Promote a positive culture focused on quality, dignity, equality, and continuous improvement.
Essential Requirements
- Minimum 2 years' experience as a Care Coordinator within a UK domiciliary care service.
- Good understanding of CQC Fundamental Standards and domiciliary care regulations.
- Experience using electronic care planning and rostering systems.
- Excellent organisational, communication, and problem-solving skills.
- Ability to work under pressure while maintaining high standards.
- Strong IT skills, including Microsoft Office.
- Full UK driving licence and access to a vehicle.
- Right to work in the UK.
Essential
- NVQ/QCF Level 3 in Health & Social Care (Level 5 desirable or willingness to work towards).
- Experience working with local authority and NHS commissioned services.
- Knowledge of capacity, consent, safeguarding, and risk management.
- Experience supporting quality assurance and audits.
