A fantastic opportunity has arisen for a fully qualified and experienced ACP, ANP or Paramedic Practitioner to join our friendly, supportive and high-achieving team at Eldene Surgery.
We are seeking a motivated and enthusiastic clinician with experience working within a Primary Care setting, who is committed to delivering high-quality, patient-centred care.
This position is available on either a part-time or full-time basis, with working hours ranging from 25 to 37.5 hours per week.
Salary: £31.16 to £34.65 per hour, depending on qualifications, skills and experience.

Main duties of the job
You will assess, diagnose and treat patients presenting with minor illnesses, providing advice and ongoing support for their continuing healthcare needs through both face-to-face and telephone consultations. Home visits are an essential part of this role.
You will deliver a high standard of patient care as part of a multidisciplinary team of Advanced Clinical Practitioners (ACPs), Advanced Nurse Practitioners (ANPs) and Paramedic Practitioners. This role requires a comprehensive clinical knowledge base, with the ability to demonstrate safe clinical decision-making and provide expert, patient-centred care.
You must be able to work both autonomously and collaboratively as part of the wider practice team.
We use Anima Triage to ensure patients are assessed and directed to the most appropriate clinician for their needs.
The ACP team benefits from regular training sessions led by our Clinical Director, as well as peer support meetings that encourage continuous professional development and shared learning.
Clinics typically consist of up to 15 patients per session, with appointment lengths of either 10 or 15 minutes.

Job responsibilities
The ACP/ANP/Paramedic Practitioner will provide an advanced clinical resource for patients, healthcare professionals, and the wider multidisciplinary team. Working autonomously and collaboratively within the practice, the post holder will deliver high-quality, patient-centred care through comprehensive assessment, diagnosis, treatment, referral, and health promotion.
The successful candidate will manage patients presenting with undifferentiated and undiagnosed conditions, including minor illness and minor injury, using advanced clinical skills and evidence-based practice. They will support patients to make informed decisions about their health, promote self-management, and contribute to the continuous improvement of services across the Practice and Primary Care Network.
A full driving licence and access to a vehicle are essential, as home visits form part of this role.
Key ResponsibilitiesClinical Responsibilities
- Assess, diagnose, and treat patients presenting with minor illness and minor injury.
- Triage patients seeking clinical advice and determine the most appropriate course of action, including referral to other healthcare professionals where necessary.
- Undertake face-to-face, telephone, and home visit consultations as required.
- Request, interpret, and act upon appropriate investigations to support clinical decision-making.
- Refer patients appropriately to secondary care services, discussing referrals with a GP when clinically indicated.
- Deliver safe, effective, and evidence-based care within the scope of professional competence, recognising and escalating limitations when required.
- Provide health promotion, education, and self-care advice to patients and their families.
- Work autonomously within professional and regulatory standards while maintaining high levels of clinical accountability.
- Maintain accurate, timely, and contemporaneous clinical records in accordance with Practice policies and professional standards.
- Support the development of colleagues through teaching, mentoring, and sharing of best practice.
- Participate in clinical audits, service evaluation, quality improvement initiatives, and the implementation of service developments.
- Contribute positively to the effectiveness and efficiency of the multidisciplinary team.
Administrative and Professional Responsibilities
- Participate fully in the administrative and professional activities of the Practice.
- Maintain accurate and comprehensive patient records using the clinical system.
- Ensure all consultations, treatments, and investigations are recorded appropriately.
- Complete all required patient documentation accurately and in a timely manner.
- Support data collection, reporting requirements, audits, and performance monitoring activities.
- Attend Practice meetings, clinical meetings, and training sessions as required.
- Maintain appropriate professional registration and indemnity cover, providing evidence to management annually.
- Comply with all Practice policies, procedures, and professional standards.
Communication and Collaborative Working
- Work closely with GPs, nursing teams, reception staff, administration teams, and other healthcare professionals to ensure effective service delivery.
- Develop and maintain positive working relationships with external healthcare providers, community services, and secondary care colleagues.
- Communicate effectively with patients, carers, colleagues, and partner organisations.
- Recognise and respond appropriately to patients' communication needs, including the use of alternative communication methods where required.
