Job description
The post holder is responsible for providing outreach mental health intervention within the homeless nursing team for hard to reach, hard to engage patients with complex, severe and enduring substance misuse and mental health difficulties.
The post holder will be responsible for providing flexible specialist face-to-face assessments, using Dialog +.
Consultations, treatment and liaison with multiple local agencies who provide support for those individuals that are homeless or at risk of homelessness.
The post holder will sometimes work autonomously within professional guidelines and exercise responsibility for their practice whilst utilising research skills for audit, policy and service development. They will contribute to policy changes within the areas supported by the service and assess the needs of service users, suggesting areas for development.
