Nottingham Mencap is looking for an enthusiastic, organised and compassionate Supported Living Coordinator to lead our Supported Living service.

Salary is £32,015.36 (full time equivalent)
Hours: 12 per week - The working week will be made up of hours working within the residential setting, and in the office. Hours may include weekend work and waking night shifts.
This is an exciting opportunity for someone who is passionate about supporting people with learning disabilities to live independently, have choice and control over their lives, and be active members of their local community.
We provide staff to support the 3 residents to live independently. We provide staff to cover waking night shifts, and support hours during the day. We provide support to maintain current and potential links within the community whilst maintaining and developing independent living skills including food preparation and attending to personal care.
You will be coordinating the service, ensuring that the service is appropriately staffed, and all documentation is up-to-date.
To run a successful accommodation-based service, it needs to be a happy place to be, which means that relationships are key. Relationships within the home between residents, staff and their local community. It is also important to ensure that relationships with family members, commissioners and social workers are positive too.
The service falls under Nottingham Mencap’s CQC portfolio, so it must be aligned with our vision to be operating as an ‘outstanding’ organisation.

b) Responsible for: Supervision of Senior Support Worker, Support Workers
c) Liaison with: Short breaks and befriending team, Lifestyle Choices, finance manager and the wider team, Carers, Commissioners and Referrers
Location:
Clarence Street in Nottingham and where the accommodation is based, Valley Road, Sherwood. We currently have one property we manage, so if this increases you may be required to work there too.
Main tasks of job:
1. Ensure that the accommodation is appropriately staffed, fully operational and planned to maximise the participation of the adults who live there. Help ensure services meet the targeted outcomes set by Nottinghamshire Care Support and Enablement Team.
2. Maintain a safe environment, including health and safety within the home, keep care plans and risk assessments up-to-date and ensure residents are well looked after.
3. To ensure support staff provide high quality support in all areas of personal, domestic and social need.
4. To directly line manage the support workers including involvement in recruitment, induction, probation, supervision, appraisals, and training ensuring all are working in a cohesive and complimentary manner.
5. In conjunction with the Senior Business Support Manager participate in the proactive management of annual leave, sickness, absence, and other personnel related issues.
6. To produce rotas to facilitate the day to day running of the service, including cover for sickness and holidays when required.
7. Create, manage, and maintain client management systems to track, monitor and forecast activity, wages, annual leave, and engagements. Production of a suite of management reports that distil the data into a format that aids proactive management of the service and integrates with financial planning.
8. Support the Senior Business Support Manager to ensure relevant documentation is completed for internal and external inspection. This may include, but is not confined to, CQC compliant standards, Risk Assessments, supervision, record keeping, Care Plans, assessment records and evaluations of care. Ensure all policy and practice meets regulatory requirements.
9. Work collaboratively with the Senior Business Support Manager and Finance Manager to accurately track financial performance. In conjunction with the Service Manager instigate remedial action plan when required to ensure services are delivered within budget.
10. Ensure that individual and group activities are sensitive to the individual needs of users and that relevant assessments (including risk) have been undertaken and communicated to support workers. Ensure effective communication with others including referrers, social workers, key workers, and carers. Establish effective feedback mechanisms.
11. Ensure the regular involvement and of residents as well as parents and carers in the development of services. Complete annual surveys to track organisational outcomes.
12. Robust implementation of policies and procedures adopted by Nottingham Mencap.
13. To preserve and respect the dignity and privacy of people and their families and observe confidentiality at all times.
14. Keep abreast of new developments in the field of Learning Disabilities. Take specific responsibility for the introduction of specific new services, improvements to current services as required and the cessation of those deemed no longer viable.
15. Embrace a culture of change and improvement. Participate in a range of measures to improve quality.
16. To proactively liaise with current and potential future funding bodies. Ensure availability of high quality and person-centred services to address the personalisation agenda.
17. Participation in relevant activities and meetings across the organisation and to act as a fully participative member of the team.
18. To participate in out-of-hours working and covering for colleague’s sickness and annual leave when required.
19. Participation in formal review of support mechanisms on a yearly basis.
