Derby Medical Centre is seeking an enthusiastic, compassionate and experienced Practice Nurse to join our dedicated multidisciplinary team.
Located in the heart of Epsom, Derby Medical Centre provides high-quality, patient-centred NHS healthcare from a modern, purpose-built facility designed for comfort and accessibility. Our experienced clinical team includes six GP Partners, five Salaried GPs, a Clinical Pharmacist, Nursing Team, First Contact Physiotherapist (FCP) and Physician Associate, all working collaboratively to deliver excellent care to our patients. We are also proud to be part of the Epsom Primary Care Network (PCN).
This is an exciting opportunity for a motivated nurse who is passionate about primary care and looking to work within a supportive and progressive practice environment. In this vital role, you will deliver comprehensive nursing care, assist with clinical procedures, and foster a welcoming environment for patients. Your dedication and clinical skills will contribute to providing high-quality healthcare services that meet the diverse needs of our community.
Responsibilities
The successful candidate will work autonomously and as part of our wider clinical team to deliver a broad range of nursing services, including:
- Chronic disease management (e.g. diabetes, asthma, COPD and hypertension)
- Cervical cytology screening
- Childhood and adult immunisations and vaccinations
- Health promotion and preventative care
- Wound care and dressings
- Travel health advice and vaccinations
- Infection prevention and control activities
- Health assessments and screening programmes
- Supporting delivery of QOF and enhanced services
- Educate patients on health management, medication usage, preventive care, and lifestyle modifications to promote long-term wellness.
- Maintain accurate and confidential medical records in compliance with healthcare regulations and practice policies.
- Support infection control procedures, sterilization processes, and equipment maintenance to uphold a safe clinical environment.
About You
We are looking for someone who has:
Essential
- Registered Nurse with current NMC registration
- Experience working as a Practice Nurse or within a community nursing setting
- Experience in chronic disease management
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal skills
- Ability to work independently and within a multidisciplinary team
- Good IT skills and experience using clinical systems (EMIS Web)
- Commitment to high-quality, patient-centred care
Desirable
- Cervical cytology qualification
- Immunisation and vaccination training
- Family planning qualification
- Experience in travel health
- Mentor or teaching qualification
- Experience leading quality improvement initiatives
- Knowledge of QOF and enhanced services
