We have an exciting role on offer within our Prevention, Provider and Support Services as a Community Care Worker in the Staying Included team.
As a Community Caseworker, you will play a key role in supporting individuals who are socially excluded, helping them reconnect with their communities and improve their wellbeing.
Working on both a 1:1 and group basis, you will:

Provide person-centred support tailored to individual needs.

Help people build confidence, resilience, and independence.
-
Enable access to community services, activities and support networks.
-
Promote the Staying Included service to stakeholders and clients.

We are looking for an enthusiastic and compassionate individual who can demonstrate:

Excellent customer service skills.

Knowledge and understanding of community support services.
-
Strong verbal and written communication skills.

A proven track record of delivering person-centred support.
-
Experience promoting services and engaging stakeholders.
To be successful in this role you need a GSCE at Grade C or 4 and above in Maths and English (or equivalent Functional Skills).
