We are a busy happy team who welcome new staff members and encourage staff development and learning. We are seeking a healthcare support worker to join us in providing care and treatment to adults living in the Fareham and Portchester area who have long term or complex health needs. The scope of your role will focus on supporting admission avoidance, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi-disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards.
To support you within your role, the Trust have an excellent learning and development department, to further develop your clinical skills.
Our service runs 7 days a week and therefore this position requires flexible working and all requests will be considered.
Access to a vehicle and possession of a full driving license is essential.

Main duties, tasks & skills required
Working under indirect supervision, as part of the Community Team, the post holder will to have responsibility for delivering nursing care to patients following prescribed care plans and protocols
Deal empathetically with sensitive information.
Safely manage self-including: -
Lone working in the community
Handling body fluids and other hazardous material
Patient handling
Medical sharps
Challenging behaviour situations
Driving self
Working as part of the Community Team and with other agencies you will provide care following a person-centred assessment for the health and well-being of patients.
To deliver nursing care by following a care plan written as part of a holistic assessment by the registered nurse for patients and their family/carers in their own home and other community settings.
Perform basic clinical activities which will include, wound care, venepuncture, blood glucose monitoring, physical observations.
To report any changes/concerns to the registered nurse on duty in a timely fashion and record in the patient's records.
Working within the Clinical Governance framework, maintain set standards and participate Standards for Better Health monitoring.
Participation in the Community Team's collaboration with other agencies, statutory and voluntary to ensure continuity of care.
To undertake basic health promotion and disease prevention activities with individuals or groups in line with the public health agenda.
