We're looking for a compassionate and proactive Social Prescribing Link Worker (MINT - Mental health and wellbeing Integrated Network Team) to join our team at Southern Brooks Community Partnerships.
This is a rewarding role supporting people across South Gloucestershire who are experiencing complex social, emotional and practical challenges that impact their health and wellbeing. Working as part of the Multi-Agency Integrated Network Team (MINT), you'll help people identify what matters most to them, overcome barriers, and connect with the services, groups and opportunities that can improve their quality of life.
As a Social Prescribing Link Worker, you will:
- Provide personalised, strengths-based support to individuals experiencing a range of health and wellbeing challenges.
- Manage your own caseload, supporting people over a series of focused sessions to identify goals and take positive steps forward.
- Use approaches such as motivational interviewing, active listening and person-centred planning to help people build confidence and resilience.
- Support individuals experiencing complex social needs, loneliness, social isolation and low to moderate mental health challenges.
- Help people access local services, community activities and support networks that improve wellbeing and independence.
- Work closely with GP practices, MINT colleagues and community organisations to provide coordinated support.
- Assess, manage and appropriately escalate risk, contributing to safeguarding and safety planning where required.
- Balance workload, demand and capacity to ensure people receive safe and timely support.
This is a varied role where no two days are the same. One day you may be supporting somebody to reconnect with their community after a period of isolation; the next, you'll be working alongside healthcare professionals and community organisations to help someone overcome barriers affecting their wellbeing.What you’ll bringWe’re looking for someone who:
- Has experience supporting people with complex issues affecting their health and wellbeing.
- Has experience managing a caseload and working independently.
- Is confident building positive relationships with individuals, colleagues and partner organisations.
- Can communicate with empathy, professionalism and compassion.
- Is organised and able to manage competing priorities in a busy environment.+1

- Believes in empowering people to achieve their own goals and make lasting positive changes.
- Can travel independently across South Gloucestershire to meet the requirements of the role.
A day in the life of a Social Prescribing Link Worker
You might begin your day reviewing referrals and planning appointments before meeting individuals in a GP practice, community venue or local hub.
Through personalised conversations, you'll help people explore what's important to them, identify barriers affecting their wellbeing and develop practical action plans. This might involve connecting somebody with a community group, supporting them to access specialist services, helping them overcome social isolation, or working alongside partners to coordinate support.
Throughout the day you'll update records, attend multidisciplinary meetings, liaise with healthcare professionals and community organisations, and ensure each person receives the right support at the right time.
By the end of the day, you'll have helped people take positive steps towards improved wellbeing, independence and connection within their community.

As a vital member of our team and in return for your passion, expertise, commitment, and helpful, empathetic approach, we will provide you with:
- A salary of £28,088 per year (pro rata)
