Full job description

An exciting opportunity has arisen for a clinical coordinator to join our Community Rehabilitation Team (CRT) based Glenrothes Hospital. CRT has the overall aim of improving the quality of care and outcomes for older people with the intention of allowing them to remain independent within their own homes or communities. A key aspect of the role is supporting the patient flow through the health system by facilitating early supported discharges from the acute hospitals and by preventing admission to hospital. This will be done by providing therapy and nursing interventions to patients on the caseload. This post is essential part of the multidisciplinary team which also includes physiotherapists, occupational therapists, nurses and healthcare support workers.

You should have excellent communication skills, be able to demonstrate sound administrative and coordinating skills required for the role, have good decision and reasoning skills, and good time management/organisational skills.

You will need to have previous NHS or similar experience working in a busy office environment and supporting the day to day functions of the team, including coordinating rota patterns and liaising with hospitals to plan discharges.

You will require to have a HNC or equivalent relevant previous administrative experience.

The ability to travel and support in other community rehabilitation team sites is essential.
Our service offers regular supervision, PDP and supports training opportunities.
