Overview
We are looking for an enthusiastic and motivated Pharmacy Technician to join our established Pharmacy Team.
Working alongside our existing Pharmacy Technician and Prescription Clerk, you will play a key role in delivering a safe, efficient and patient-focused medicines management service. You will work closely with GPs, nurses, care coordinators and administrative colleagues to ensure patients receive the right medicines at the right time while supporting safe prescribing and continuous improvement across the practice.

Main Responsibilities
Medicines Optimisation
Undertake medicines reconciliation following hospital discharge and other transfers of care within agreed practice timescales.
Maintain accurate medication records on the clinical system, updating and reviewing medications through agreed practice pathways and following changes arising from medicines reconciliation, medication reviews and clinical correspondence.
Undertake agreed searches and recalls for patients requiring high-risk drug monitoring, medication reviews and to support achievement of QOF and other contractual requirements.
Support medication review processes by identifying patients requiring review and preparing information for clinicians in accordance with practice protocols.
Support safe, evidence-based and cost-effective prescribing through medicines optimisation initiatives.
Repeat Prescribing
Process repeat prescription requests accurately, safely and in accordance with practice SOPs and agreed turnaround times.
Convert appropriate acute medicines to repeat medication in accordance with practice protocols.
Support and manage Electronic Repeat Dispensing (eRD) processes, identifying suitable patients and promoting uptake in line with NHS guidance and practice protocols.
Support the preparation and submission of prescription claim information, ensuring relevant prescriptions are identified, processed and recorded accurately in line with NHS requirements.
Identify opportunities to improve repeat prescribing systems and SOPs.
Patient Care
Respond to medication queries promptly, documenting actions and escalating clinical issues appropriately.
Provide advice within professional competence and promote positive patient experience.
Quality Improvement
Take ownership of allocated medicines management workstreams and ensure these are completed within agreed timescales.
Participate in prescribing audits, Ardens searches and QOF-related medicines optimisation work.
Identify and contribute to service improvements to improve prescribing systems, patient safety and workflow efficiency, bringing suggestions forward to the Pharmacy Team and Management Team.
Governance
Complete emergency medicines and controlled stationery checks.
Support training of colleagues and comply with legislation and practice policies and standard operating procedures.
Professional Responsibilities
Maintain GPhC registration.
Participate in CPD, appraisal and mandatory training.
Maintain confidentiality and comply with GDPR and information governance requirements.
Comply with safeguarding, information governance, infection prevention and control, and health and safety requirements.
Undertake any other duties appropriate to the role and grade of the post.

Person Specification
Essential
Registered Pharmacy Technician (GPhC)
Level 3 Pharmacy Technician qualification
