Specialist Physiotherapist – Liver Reconditioning Service
We are excited to offer an opportunity for an expereinced and motivated Band 6 Specialist Physiotherapist to join our innovative Liver Reconditioning Service in Cardiff and Vale UHB, supporting patients with advanced liver disease to optimise their health and independence from home. A team of dietitians, nurses and a physiotherapist work in collaboration with the inpatient hepatology team to identify, assess and treat patients who may benefit from a home-based nasogastric feeding and reconditioning pathway, providing outreach care.
You will be required to:
Deliver specialist physiotherapy assessment and intervention to patients with complex liver disease in community settings
Design and implement individualised reconditioning programmes to improve strength, frailty and functional independence
Work closely with MDT colleagues to monitor clinical progress, outcomes and patient safety
Support patients to maximise independence in meaningful activities such as work, family roles and physical activity
Contribute to service development, evaluation, and the establishment of best practice models of care
We are looking for a physiotherapist who:
Has post-registration experiences at Band 5/6 level, ideally including complex medical, frailty, or gastroenterology/hepatology patients
Demonstrates strong skills in rehabilitation, functional assessment and patient-centred care
Is confident working autonomously in community settings.
Has excellent communication, MDT and MI skills.
Is motivated to contribute to an innovative and evolving service
Experience in chronic disease management is advantageous
