Extended Hope is a support service for young people in mental health crisis in Surrey. It is based in a small children's home in Redhill but covers the entire County. This home provides a short-break service for up to two children who experience social, emotional and mental health difficulties. It is operated by Surrey County Council and the local NHS trust. Other interventions provided include after hours contact by arrangement for young people and families and some in-reach to acute hospitals when there are admissions for mental health concerns.
Main duties include the assessment of, and support for, children in mental health crisis and assisting local care networks to effectively plan and execute intervention plans. Other focus areas are concentrated on being an impactful member of the team (along with Residential Support Workers) providing direct care for young people who are staying in the house during respite breaks.

Job Purpose
- Report to the Manager of Extended Hope and provide leadership in the development and delivery of service.
- To provide timely assessment of young people who present in mental health and social care crisis within the acute hospitals, Health Based Place of Safety (HBPoS) or other community spaces across surrey. Young people may present with deliberate self-harm behaviour, suicidal ideas, suicide attempts, family breakdowns, complex family dynamics, breakdown of adoption, foster and children home placements. Assessments of the above presentations with appropriate signposting, care planning, safety planning, risk formulation/management & community planning applied.
- To work collaboratively with the Crisis pathway provision within CYPS and Social care and offer community-based assessments with emergency services when there could be a possibility of supporting young people in their communities as an alternative to being placed on a section of the Mental Health Act (MHA) or removed to a place of safety.
- To provide support, advice and education to the Emergency Departments, Paediatric Wards or HBPoS to build knowledge and skills of staff within these settings in the care and treatment of young people. Our hospital colleagues will be involved in care provision and planning throughout any Paediatric or HBPoS inpatient journey.
- To work closely with the CYPS Crisis Pathway to provide a streamlined response to meet the needs of young people and their family/carers. This includes the Crisis Intervention Team, Paediatric Liaison Service, Hope Service/Hope Day Programme, Adolescent Team, Childrens Crisis Service (CCS provision SCC) Youth Workers and the Emergency Duty Team (EDT).
- To be part of the Mindworks provision within Surrey aiming to deliver a Thrive approach to care and treatment. This is a partnership between CYPS (formally known as CAMHS) and the Alliance partners.
Key Responsibilities
- To work closely with Mindwork services during office hours, responding to emergency referrals of young people presenting in crisis.
- Work as an autonomous practitioner, undertaking clinical assessment of young people who may have complex co-morbid mental health and/or social care needs and present in escalating crisis. You will be working alongside medical, nursing, social care and other professional colleagues to ensure that the holistic approach to meeting the needs of the young person and family/carer is considered. As a practitioner at the Extended Hope Service you will consider how your service can complement the short-term plan that can allow the least restrictive options for the young person at that time.
- To co-ordinate and liaise with ED, Paediatric wards, PLNs, or HBPoS when young people with mental health and/or social care needs attend Acute General Hospitals or are admitted to the Paediatric wards or HBPoS. As this role in out of hours you will have access to senior colleagues should support regarding decision making be required.
- Work closely with the Emergency Departments, CYPS Crisis Line, Paediatric Triage Line, the Adult and CYPS Psychiatric Liaison Services, Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, ensuring young people are seen in a timely manner and risk is assessed and a management plan is developed.
- To work alongside the Hope Community service to support young people who are required to stay (for emotional or psychological reasons) in acute care settings through in-reach visits and engagement insitu. This will be an adjunct to the work of that service and constitute an additional responsibility in alignment with that service’s Care Coordinators.
- To support formal Mental Health Act assessments of young people when required, in order to guide care and treatment planning by AMHPs and s12 Drs.
- To manage own caseload from referral to discharge/transfer, holding medium to high risk/needs cases.
- Ensure that outcome measures for the service are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.
- Be responsible for the initial assessment and subsequent facilitation of admission to the Extended Hope House Respite facility and onward referral to Hope or other services and discharge to appropriate services.
- All aspects of care planning, risk formulation & psychotherapeutic care are recorded on Systmone (electronic patient record). Care plans and My Safety Plans are produces in collaboration and signed by young person and family/carer.
- Ensure any safeguarding referrals are completed, sent and followed up to ensure response to concerns are recorded.
- Work closely with the Mental Health Support Workers who care for young people admitted for respite and offer training opportunities for their development.
- Report to and advise referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies.
- Represent the Extended Hope Service at professional/network meetings to offer advice/consultation on complex cases who are in crisis.
- Develop and implement systems to monitor and evaluate the service.
- You will be expected to be involved in appraisal for any one you are formally supervising – appraisals will be led by the Manager.
- To undertake relevant CPD to improve assessment and treatment skills regarding the care of young people with presenting mental health and social care needs.
- Be competent in comprehensive assessments, risk formulations and risk management plans aka My Safety Plans.
- Share skills & knowledge and offer informal training to wider network including social care orientated professionals and emergency service departments.
- To provide assessment of young people in complex family / social & mental health crisis with the aim of admission to Extended Hope House.
- Attend the Crisis Pathway Learning sets that are multi-agency in nature.
You will have monthly supervision and an annual appraisal that will ensure you have access to development opportunities and that consider your career pathway. This can involve the Nurse Consultant for Crisis Pathway or the CYPS Head of Nursing at appraisal so your career progression and support is fully considered
