The Steps to Support (STS) program provides community-based mental health and housing stabilization services to
individuals living in Toronto Community Housing. Using a Housing First, trauma-informed, recovery-oriented, and
client-centred approach, the program promotes housing stability, enhances quality of life, and supports individuals
in maintaining successful tenancies.

Serving approximately 420 tenants annually through a hub-and-spoke service model across multiple Toronto
Community Housing communities, the program also delivers low-barrier services that reach more than 3,000
tenants each year.

As a Housing Stabilization and Support Specialist, you will work directly with tenants experiencing complex mental
health, substance use, and housing challenges. This is a highly mobile, community-based role where you will build
trusting relationships, provide individualized case management, respond to crises, coordinate services, and help
tenants maintain stable housing and achieve their personal goals.

What You Will Do:

Build trusting, respectful relationships with tenants through consistent engagement and individualized support.
Complete comprehensive intakes, needs assessments, goal planning, and ongoing case management using

client-centred, strengths-based, and trauma-informed practices.

Develop and implement housing stabilization plans that support successful tenancies and reduce the risk of

housing loss.

Respond to mental health, substance use, behavioural, and housing-related crises using de-escalation, conflict

resolution, and sound professional judgment.

Coordinate services with Toronto Community Housing, healthcare providers, income support programs, legal

services, and community agencies.

Support tenants in accessing healthcare, income assistance, food security programs, and other community

resources.

Accompany tenants to appointments and advocate on their behalf when appropriate.
Coach tenants in developing life skills such as budgeting, self-care, maintaining healthy living environments, and

community participation.

Support tenants during housing transitions, including preparing for unit inspections, coordinating moving

supports, and promoting long-term housing stability.

Facilitate or co-facilitate group programming that promotes well-being, life skills, and community connection.
Maintain accurate, timely, and confidential documentation in accordance with organizational and privacy

standards.

Provide regular updates to the Manager regarding client progress, emerging risks, and service needs.
Participate in team meetings, supervision, case conferences, training, and ongoing professional development.
Support the development of community partnerships and new programming opportunities.
Represent Fred Victor professionally with tenants, partners, and stakeholders.
Perform other duties as assigned.

What You Bring to the Team:

Diploma in Social Service Work, Social Work, or a related field, or an equivalent combination of education and

experience.

Minimum three to five years of experience supporting individuals experiencing mental health challenges,

substance use, homelessness, or housing instability in community-based settings.

Experience providing case management and housing stabilization services; experience in supportive housing is

considered an asset.

Strong knowledge of Housing First principles, harm reduction, recovery-oriented practice, trauma-informed

care, and eviction prevention strategies.

Demonstrated experience responding effectively to crises using de-escalation and conflict resolution techniques.
Knowledge of housing, healthcare, income support, and community resources throughout Toronto.
Strong assessment, problem-solving, case planning, and supportive counselling skills.
Excellent organizational, documentation, and time-management skills with the ability to manage competing

priorities.

Excellent verbal and written communication skills.
Proficiency with Microsoft Office and electronic client information systems.
Ability to work independently while collaborating effectively within a multidisciplinary team.

Working Conditions:

Work is primarily conducted within community-based supportive housing environments serving a diverse tenant

population, including families, seniors, and individuals experiencing complex mental health, substance use,
health, and social challenges.

Regular interaction with individuals experiencing crisis, housing instability, and other complex circumstances

requires sound judgment, strong situational awareness, and the ability to remain calm and effective under
pressure.

Respond to critical incidents and high-risk situations, including medical emergencies, behavioural crises, and

community safety concerns, in accordance with organizational policies and procedures.

The role involves supporting individuals experiencing poverty, mental health challenges, substance use, social

isolation, and systemic barriers, requiring resilience, professionalism, and a trauma-informed approach.

Work includes on-site support, community outreach, accompaniment to appointments, meetings, and

coordination with internal and external partners.

This position is scheduled Monday to Friday, 9:00 a.m. to 5:00 p.m., with occasional evening or weekend work to

support community events, stakeholder engagement activities, or critical incident response.

Compensation and Benefits:

For the Housing Stabilization and Support Specialist, Fred Victor offers an hourly rate of $31.30
7% vacation pay
