Job summary
We are seeking an enthusiastic, hardworking and motivated Occupational Therapy Associate Practitioner to join our Lymington Stroke Therapy Team. You will support the multidisciplinary team to deliver a high quality and efficient service to stroke patients across the New Forest.
The role involves completing specified assessments and treatment programmes, working with patients in the community on a one to one or group basis to support rehabilitation exercises and functional activities. You will be supported by Clinical Specialist Team Leads and highly experienced Senior Occupational Therapy staff. The post involves close working with the Occupational Therapists, as well as, Physiotherapists, Speech and Language Therapists, CST Nurse and other team members.
Applicants should have experience as a therapy assistant, with some knowledge of neurology relating to stroke. The successful candidate will complete stroke specific competencies and receive relevant training, while participating in supervision, service development, the IST programme and a weekend rota supporting our 7day service.
A full driving licence and access to a vehicle for work are essential.

Main duties, tasks & skills required
To be responsible for the holistic assessment of patients new to the service, as well as on-going assessment of service users on the caseload. It includes the consideration of the clinical, social and psychological needs of both the service user and their carers.
To undertake specified assessments of patients, to interpret assessment information and plan patient care using judgemental skills to decide upon and recommend best course of action within competencies / with supervision.
To formulate an individual management and/or treatment programme, usingclinical reasoning skills and a range of treatment skill within competencies.
To progress treatment programmes with specified limits as agreed with senior staff.
To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their personalised goals, optimum level of independence and quality of life, as directed by the team of therapists/nurses. Working with the individual patient or in groups.
To implement specific treatment programmes as specified by team therapists and nurses to stroke patients in the community.
To advise and teach relatives, patients and carers on the routine and standard management of Stroke including secondary prevention to promote understanding of the aims of intervention and to ensure a consistent approach to patient care.
To participate in completing routine (non-complex) 6 week and 6-month stroke reviews for stroke patients within the New Forest with a Hampshire GP.
