This is an exciting opportunity for Senior Health Care Support Workers to join our HART Team and support our NEW virtual ward.
The HART (Hospice Advice, Response and Team) is already a well-established and valued multidisciplinary service, delivering an advice line and rapid response service to support patients, families, and healthcare professionals.
Working as part of a highly skilled multidisciplinary team, you will help deliver safe, timely and compassionate care at home, supporting telephone advice, rapid response activity and face-to-face assessments where required. Guided by clinical nurse specialists, specialist paramedics and medical practitioners, you will provide hands-on care, undertake patient observations and assessments, support agreed care plans, and contribute to the smooth coordination of patient access and flow across the Virtual Ward and wider community services.
About You
We are looking for someone who is experienced, compassionate and confident who can respond calmly and effectively in time-critical situations. You will be a strong communicator, able to work collaboratively with colleagues, patients, families and healthcare professionals.
The successful candidate will be flexible, proactive and organised, with the ability to adapt to changing priorities, support coordinated care across services and maintain high standards of dignity, safety and comfort for patients with complex palliative and end-of-life care needs.
Key Responsibilities

Provide hands on care and support to help patients in accordance with agreed care plans

Provide telephone and face-to-face support to patients and families, including bereavement calls

Observe, report and escalate changes in patient condition appropriately

Support referrals and audits to enable holistic patient care

Maintain accurate, timely and confidential patient records

Work collaboratively as part of a multidisciplinary community team

Attend mandatory training and palliative care education

Salary: £26,091 + Anti-Social Pay
Hours: 22.5, 30, 37.5 contract hours available.
Hours 8am – 4pm or 12pm - 8pm over 7 days (some flexibility required to meet service demands, there is a rotational requirement for night working)
