Job Summary
PCN Care Co-ordinators play an important role providing extra time, capacity and expertise to support patients to be actively involved in managing their care and supported to make the choices that are right for them. Working closely with GPs, GP practice teams and other health and care professionals and organisations involved in care, they act as a central point of contact for patients.
This role is intended to become an integral part of the PCN’s multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to promote and embed a personalised care approach.
You must be able to work independently as well as part of a team, be a forward thinking, caring & dedicated A reliable and person-focused individual who enjoys working with a wide range of people. You will have a strong working knowledge of Office 365 programs and general IT, good written and verbal communication skills and proven strong organisational and time management skills. As this is an evolving role, we are looking for highly motivated and proactive applicants with a flexible can-do attitude, keen to work and learn as part of a team, and who will be committed to providing patients and practices with high quality support.
Although this is predominantly a remote working role, you will be expected to attend in person, Mandatory monthly team meetings, and PCN Meetings. You may also be asked to support at a group session at short notice. Due to the locations of our surgeries, you will need access to your own transport. The post holder will demonstrate excellent organisational and communication skills, be flexible in their approach, able to exercise initiative and demonstrate consistently high standards of professionalism. The post holder must always be aware of the need for confidentiality and integrity. A basic knowledge of Health and Social Care terminology, structures and pathways would be a strong advantage.
Please note that the role of a care coordinator is not a clinical role.
Mission Statement
GP practices, grouped into Primary Care Networks (PCNs), are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas to form Integrated Neighbourhood Teams. This role will include lots of contact with these organisations.
Primary Care Networks (PCNs) and Integrated Neighbourhood Teams are a new way of working that enables health and other services to work together to provide better access for patients.
PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.
We are hoping successful applicants will help us create new ways of working with partner organisations – we need someone highly motivated, keen to help shape how communication can be improved between partner organisations to improve patient care.
Primary Responsibilities
The following are the core responsibilities of the PCN Care Co-ordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- To identify cohorts of patients and signpost to the relevant services.
- To support the coordination and administration of all Meridian clinics i.e. Weight Management, Menopause & Pain Management sessions.
- Adhoc administration tasks and clinic booking for GP practices.
- Ensuring patients records are updated and correctly coded.
- Support the coordination and administration of MDT meetings, taking and distributing minutes, bringing information and feedback to others where needed within Meridian PCN.
- Administrative support for all PCN Projects.
- To reduce administrative burden on GP’s and other clinicians.
- To attend the MAF (Multi Agency Forum) meetings when needed and have information ready for this
- Check coding is correct in SystmOne & Emis
- Begin the Patient Passport with Proactive Neighbourhood Patients

Qualifications: Essential
Good general education including core level of Maths and English
