About the role
Overview of the Learning Disability Team/Service:
The Learning Disability Team provides specialist support to adults with learning disabilities, ensuring that they receive the right care, support, accommodation, education, and health services to achieve positive outcomes. The service works collaboratively with individuals, families, carers, health professionals, education providers, and community organisations to promote independence, wellbeing, safety, and inclusion.
Impact of the Role on the Organisation
The role within the Learning Disability Team has a significant impact on both the organisation and the individuals it serves. It contributes to:
Improved outcomes for individuals and families by ensuring timely assessments, interventions, and support arrangements. Safeguarding and risk management, protecting vulnerable individuals and ensuring statutory responsibilities are met. Efficient use of resources through effective commissioning, care planning, and placement management.
Service improvement and quality assurance by monitoring performance and driving best practice. Partnership working, strengthening relationships with health, education, housing, and community services. Reduced placement breakdowns and crisis interventions through proactive support and robust planning.
Successful transitions and independence planning, helping individuals achieve greater autonomy and community inclusion. Compliance with statutory duties, ensuring the organisation meets its legal obligations under relevant social care and equality legislation.
This is a part-time opportunity, working 26 hours per week.

What will you be doing?
Day to Day tasks and duties include (but are not limited to) the following:
Assessing the needs of people with learning disabilities and their families. Developing and reviewing care and support plans. Safeguarding vulnerable adults.
Attending the Quality Assurance Forum to request for placements and support packages. Working in partnership with health services, housing, and voluntary organisations. Promoting independence and reducing reliance on long-term institutional care where appropriate.
Ensuring compliance with relevant legislation, policy, and best practice. Undertaking duty and completing duty tasks. Attending the MDTs.
About you
Skills and Experience:
Strong assessment and care planning skills, with the ability to identify needs, strengths, risks, and desired outcomes. Excellent communication and interpersonal skills, enabling the development of positive relationships with service users, families, carers, and partner agencies.
Effective safeguarding knowledge and the ability to recognise, assess, and respond appropriately to safeguarding concerns. Strong organisational and time-management skills, with the ability to manage competing priorities and a complex caseload.
Ability to undertake strengths-based, person-centred assessments and reviews. Effective report writing and record-keeping skills, ensuring accurate and timely documentation. Problem-solving and decision-making skills, particularly in complex situations requiring multi-agency involvement.
Ability to work collaboratively within multidisciplinary teams and across partner organisations. Good IT skills and experience using electronic case management systems. Ability to work both independently and as part of a multi-disciplinary team. Ability to work from a variety of locations and travel countywide independently.
