Care Worker – Outreach

“I’ve noticed a difference since LifeCare visits started. When I call Dad, he is more upbeat and chattier than he has been in ages.”

28-35 hours per week between 8am and 6pm Mon-Fri.
£13.71 per hour, above Real Living Wage.
Experience in care or similar role, is essential.
Appointment subject to satisfactory PVG and references.
Requirement to be or to become registered with SSSC.
Being a driver with access to a car advantageous, but not essential.
Sponsorship cannot be supported.
Must have UK Right to Work.
Applications from people living in Central or East Edinburgh would be particularly welcome but not essential.

OUTREACH SERVICE

LifeCare is an Edinburgh based charity that’s here to give Edinburgh’s older people extra years of zest. We don’t believe in rushed, 15-minute home visits. We offer nothing less than an hour. Which helps you do what you do best: care, in the true sense of the word.

Our registered Outreach service works across the City of Edinburgh.

Our Outreach service supports people to live in their own homes and be independent for as long as possible. You will make a big difference to people’s lives by offering the highest quality personalised and responsive care, companionship, and opportunities to participate in community life, as well as assisting with household tasks and providing personal care where required.

POSITION IN ORGANISATION

Reports to: Service Senior or Co-ordinator, Outreach Service

KEY RESPONSIBILITIES

1 Client and Carer Wellbeing, Support and Care

Work as part a team providing an excellent care service for older people and/or their carer in order to support independence. To promote Extra Years of Zest and enhance their quality of life and provide the support to do the things that are important to them.

Ensure that the service delivered:

provides for the emotional, social, and physical well-being of clients and/or carers.
demonstrates respect for people’s privacy, dignity, choice, independence, rights and fulfilment and values people as individuals.
meets best practice standards of service delivery.
encourages the exercise of choice by clients and carers.
Liaise with the Service Senior or Co-ordinator on all issues to do with client care and carer support.
Work with individual clients and carers assigned to you as their key worker and implement and review care plans with them.
Ensure any changes or relevant information is captured fully and written in the client’s care plan and that visit reports and risk assessments are recorded accurately and as soon as possible. Report any changes or emerging concerns to the Service Co-ordinator.
Provide a service that meets clients’ and carers’ desired outcomes and the clients’ welfare, as reflected in their care plan.

2 Quality Assurance, Legislation & Regulation

Work to LifeCare’s policies and procedures that govern the provision of care services to meet Health and Social Care Standards, SSSC Codes of Practice and which comply with Health and Safety.
Employ safe working practices and report anything untoward to the Service Co-ordinator as soon as it occurs.
Alert the Service Co-ordinator to any comments, suggestion and complaints from client’s, carers, or others.

GENERAL RESPONSIBILITIES

Work flexibly.
Engage in opportunities for continuous professional development.
Abide by organisational policies, codes of conduct and practices including equality and diversity.
Treat with confidentiality any personal, private, or sensitive information about clients, carers, volunteers, other staff and the organisation in accordance with LifeCare’s GDPR policy.
