We are delighted to offer an exciting opportunity for a Outreach and Recovery Support Coordinator in Warrington. You will use your skills and experience to assess, engage and empower Service Users, ensuring they access the right support at the right time, in a way that truly meets their needs.
With this role you’ll be working as part of the Pathways to Recovery team based in Warrington. The team supports people with their or their loved ones’ drug and/or alcohol issues.

Full Time Hours: 37.5 hours per week
Contract Type: Permanent
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities:

Focus on early interventions for clients entering the service, specifically as facilitator via online group work interventions.
- Follow the organisations infection control guidance is adhered to including delivering COVID screening and testing as required.
- Be able to work autonomously, safely and creatively, using your own initiative away from the office

Work highly collaboratively with other organisations on behalf of service users.
- Accurately record and input client data and information to fulfil contractual, administrative, and financial requirements.
- Develop a personalised action plan with each service user in conjunction with other agencies and professionals as required.
- Provide, detached, domiciliary and peripatetic outreach work.

Carry out dry blood spot testing with a view to reducing BBV rates.

Providing screening, assessment and recovery planning and onward referral
- Reducing drug and alcohol related harm to service users and the wider community.
- To work collaboratively with key partners in developing multi-agency assessment and care plans to meet the holistic needs of individuals.
About You:

Possess knowledge and experience in delivering interventions for service users affected by opiate use.

Excel in both verbal and written communication, with proficient IT skills.

Demonstrate proactive qualities, working independently with excellent time management and prioritisation skills.

Hold a strong understanding of mental health interventions, services, and best practices.

Thrive as a supportive team player with exceptional interpersonal skills, capable of collaborating with a diverse range of professionals, agencies, and internal/external stakeholders.

Exhibit the ability to navigate change effectively, always prioritising the needs of service users.

Recognise the significance of information governance processes and commit to adhering to all necessary safeguards.
