We have an exciting opportunity for a Homeless Outreach Worker to work within our Manchester Service, engaging with people experiencing rough sleeping with the aim of moving them into more settled lifestyles.

Hours: Full time, 37.5 per week
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities:
As part of the Outreach Team, you’ll help people get to where they want to be and make a difference in their lives.
You will work directly with individuals experiencing homelessness and substance misuse with the aim of building trusting relationships on the streets, that facilitates the provision of harm reduction interventions and encourages entry into structured drug/alcohol treatment. You will work in collaboration with other professionals such as colleagues from the Manchester Homeless Partnership, and act as a vital bridge between accommodation and other partnership services.
Enjoying a bit of paperwork and attention to detail is important as you will have to record information about the interventions you provide and you will complete assessments/risk reviews, facilitate medical appointments with the clinical team and support individuals to develop their individualised recovery plan.
No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key. Some knowledge of one or more of the following is required:
Homelessness
Mental health
Criminal Justice System
Substance misuse
