Housing Practitioner

Job Type: Permanent

Hours: 30 hours per week

About Catch Scotland

Catch Scotland provides tailored housing support, vocational training, care and learning experiences for care-experienced young people aged 16–25. Our aim is to support young adults to develop the skills, confidence and resilience needed to live independently and achieve positive outcomes.

We are looking for compassionate, reliable and committed individuals to join our team as a Housing Practitioner.

Purpose of the Role

To support the Community Supervisor with the day-to-day operation of up to three Young Adults' houses and respond promptly and effectively to any issues that arise.

The Housing Practitioner will support the delivery of services in accordance with relevant National Care Standards, Care Inspectorate requirements and organisational policies and procedures. The role promotes Young Adults' rights, responsibilities and independence through a person-centred approach that encourages self-awareness, personal growth and positive outcomes.

Reporting Line

The Housing Practitioner reports directly to the Community Supervisor.

Working Pattern

Please ensure you can commit to the rota below before applying.

Weekly Pattern

One office day each week (Monday - Friday)
One backshift/sleepover shift each week (Monday–Thursday)
Rolling Weekend Rota (4 week rotation)

Rolling Weekend Rota

Week 1: Friday sleepover shift

Week 2: Saturday shift

Week 3: Sunday shift

Week 4: Weekend off

The rota then repeats on a four-week cycle.

Flexibility of Hours and Duties

The Employee's normal working pattern is outlined above. However, due to the evolving needs of the service, operational requirements, the needs of Young Adults and organisational development, Catch Scotland reserves the right to reasonably amend working hours, shift patterns, duties, responsibilities and areas of work following consultation with the Employee.

Key Responsibilities

The duties listed below are not exhaustive and may be amended from time to time to meet the operational needs of the service.

Case Management

Be responsible for allocated Young Adults' case files.
Ensure all records remain accurate, up to date and compliant.
Notify relevant managers of significant changes as required.
Maintain and review:
Care Plans
Information Sheets
Risk Assessments
About Me Documents
STAR Assessments
Housing Meeting Records

Reporting and Record Keeping

Complete weekly and monthly progress reports within agreed timescales.
Ensure reports are solution-focused, goal-oriented, evidence-based and professionally written.
Review weekly and monthly logs, ensuring all staff complete documentation accurately and within required timescales.

Meetings and Communication

Arrange Housing Meetings for allocated Young Adults and properties.
Attend meetings and undertake minute-taking duties as required.
Distribute relevant information and actions arising from meetings to appropriate parties.
Maintain effective communication with Young Adults, colleagues and external professionals.

House Management

Support Young Adults to maintain a safe, clean and welcoming living environment.
Ensure household standards are maintained.
Support Young Adults to manage appointments and repeat prescriptions, encouraging independence and self-management wherever possible.
Create and maintain household chore rotas.
Coordinate shopping activities with Young Adults and allocate support tasks to staff where required.
Ensure Young Adults are aware of staff on duty through the use of communication boards and other agreed systems.

Team Support

Work collaboratively with the Community Supervisor to support the effective operation of Young Adults' houses.
Allocate and coordinate tasks amongst support staff to ensure agreed outcomes and goals are achieved.
Provide guidance and support to colleagues in line with organisational policies and procedures.

On-Call Duties

Participate in the organisation's on-call rota as required.
Respond appropriately to emergencies and operational issues when on call.
Escalate concerns to the Community Supervisor or Senior On-Call Manager where necessary.

Full UK driving licence

Registration with the SSSC within three months of appointment

Location

Based across Renfrewshire, West Dunbartonshire and Inverclyde, supporting young people in their own homes. Easily accessible from Glasgow, Paisley and surrounding areas.
