Job Title: Prescribing Minor Illness Practitioner

Maintain registration with professional body working within the Code of Professional Conduct.
Hours: 30-37.5 hours per week
Pay: Salary competitive depending on experience
Location: Wooda Surgery
Training
· To participate in all training and supervision requirements outlined by the practice.
· To meet the requirements of Prescribing Frameworks.
· Demonstrates a commitment to lifelong learning and is accountable for managing own CPD, maintaining professional registration requirements, and ensuring ongoing clinical competence.
2. Job Purpose
To provide safe, effective, and patient-centred clinical care within a Primary Care setting. The post holder will assess, diagnose, treat, prescribe and appropriately refer patients presenting with a wide range of acute and chronic health conditions.
The role includes wound care management, diagnostic assessment and interpretation, infection prevention and control adherence, and independent clinical practice working collaboratively with the multidisciplinary team to improve patient outcome
Diabetes and Respiratory disease management with opportunities for further development in these areas supported by the practice.

4 . Main Tasks/Duties and Areas of Responsibility
Clinical Responsibilities:
· Undertake comprehensive clinical assessments of patients presenting with acute and undifferentiated conditions.
· Diagnose, treat and manage patients within own scope of professional practice.
· Independently prescribe medications in accordance with national and local prescribing guidelines.
· Order and interpret diagnostic tests and investigations.
· Develop management plans and provide health education and self-management support.
· Identify when referral to another healthcare professional or service is appropriate.
· Assess, diagnose, treat and manage patients presenting with minor injuries and acute wound care needs within own scope of practice and clinical competence.
· Undertake wound assessment, dressing selection, wound management planning and ongoing review in line with local and national clinical guidance.
· Support the nursing team in the delivery of childhood immunisation programmes, including undertaking immunisations where appropriately trained and competent to do so
· Support the provision of cervical screening services and undertake cervical cytology sampling where appropriately trained, competent and compliant with national cervical screening programme requirements where appropriate.
· Provide face-to-face, telephone and remote consultations as required.
· Undertake home visits where clinically appropriate.
· Maintain accurate and contemporaneous clinical records using the practice clinical system.
· Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, in line with national and local policies and practice needs.
· Support medicines optimisation initiatives.
· Support development of programmes to improve health and well-being and meet the specific needs of communities, groups, and individuals.
· Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures
· Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance
Supervision
Support the team (Practice Nurses and HCAs) with the day-to-day delivery of treatment room services, including wound care management, infection prevention activities, chronic disease monitoring and health promotion initiatives

They may also have access to information relating to the Practice as a business organisation. 5. Hours of work
Extra hours of work:
Staff will be required to work additional hours to provide cover for annual leave, occasional study leave and unexpected absences. Extra cover is paid at the normal hourly rate or taken as TOIL.
Attendance at meetings and training sessions held throughout the year will be paid at your normal hourly rate if held outside your working hours.
Weekends:
Staff are expected to participate in the practice's seasonal vaccination programmes, including weekend Flu and COVID vaccination clinics, on a rotational basis as required by service need. Attendance at these clinics will be remunerated at time and a half (1.5x) the normal hourly rate.
Staff are also expected to support the delivery of Extended Access services on a rotational basis. Hours worked during Extended Access sessions will be remunerated at time and three-quarters (1.75x) the normal hourly rate at the discretion of the Partners.
Participation in these services forms part of the post holder's commitment to supporting patient access and continuity of care across the practice population.
Bank holidays:
There may be a rare occasion when staff are required to work a bank holiday and this would be paid at double your hourly rate
