The role:
Inverclyde HSCP ANP/Trainee ANP service delivers urgent and unscheduled care on behalf of GP Practices through a dedicated home visiting model. Responding and supporting care homes residents, housebound patients and those patients too acutely unwell to visit their GP Practice; the service ensures timely, accessible care where it is most needed.
Working in close partnership with GPs and multidisciplinary teams, the service provides seamless, high-quality, and safe care within the community. ANPs collaborate with carers, care-at-home services, care home staff, and social care colleagues to deliver patient-centred support tailored to individual needs. The service manages a wide range of conditions, addressing physical and psycho-social needs of this population. ANPs carry out comprehensive clinical assessments, arrange investigations, develop treatment plans, prescribe where appropriate co-ordinate the transfer of patients to hospital when required.
The service plays a key role in health education and promotion, supporting rehabilitation, enhancing wellbeing and helping patients maintain independence in their home for as long as safely possible.
All roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Duration, Location, and Working Pattern:
There are 2x permanent posts available:
1x full-time at 36 hours per week
1x part-time at 27 hours per week.
Please note the salary is pro Rata for part-time hours.
The shift patterns will be discussed with the successful candidates; however, the shift will commence at 8.30am
You will be based at Port Glasgow Health Centre, Bay Street, Port Glasgow
You will be expected to work across roles and geographical boundaries, collaborating with a range of health and social care professionals throughout Inverclyde.
Key responsibilities:
ANP’s are educated at Masters Level in advanced practice and are assessed as competent in this level of practice. As a clinical leader they have the freedom and authority to act and accept responsibility and accountability for those actions. This level of practice is characterised by high level of autonomous decision making, including assessment, diagnosis, treat, including prescribing for patients with complex multi-dimensional problems. Decisions are made using high level expert knowledge and skills. This includes the authority to refer, admit and discharge within appropriate clinical areas.
ANP’s have developed a highly specialist knowledge across a range of work practices and procedures underpinned by theoretical and practical knowledge and experience.
The successful candidate should be:

Recorded as a Nurse Independent/Supplementary Prescriber (V300) with the NMC.

Working towards/committed to working towards* or have an appropriate Masters level qualification in Advanced Practice which has enabled competence in: · Clinical assessment · Clinical reasoning, judgement and diagnostic decision making · Anatomy and pathophysiology · Non medical prescribing- V300 · Leading, delivering and evaluating care · Practice learning/transferable work based learning

Advanced Nurse Practitioners (ANPs) are Master’s level clinicians with the autonomy, authority, and accountability to assess, diagnose, treat, and prescribe for patients with complex needs. Using advanced clinical judgement, they manage care independently, including referral, admission, and discharge decisions, supported by specialist knowledge and experience.
Applicants must be NMC-registered Independent/Supplementary Prescribers (V300) with a Master’s qualification in Advanced Practice and relevant clinical experience; they should have successfully completed Clinical assessment, Clinical reasoning, judgement and diagnostic decision making, Anatomy and pathophysiology, Non-medical prescribing- V300, Leading, delivering and evaluating care and Practice learning/transferable work based learning - community experience is desirable. An interest in palliative care, long-term conditions, and anticipatory care is advantageous.
A full driving licence is essential to support delivery of care to patients across community settings.
