Main duties of the job To practice as an emergency care practitioner and demonstrate advanced clinical competence and knowledge above those associated with conventional nursing roles. Assess, plan, treat and refer patients appropriately, carrying out consultations in the surgery or the community as an autonomous practitioner. Using own clinical judgement to diagnose and treat patients either face to face consultation, clinical telephone triage and other mediums. Support patients to adopt health promotion strategies to improve healthy lifestyles, and apply principles of self-care. Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols. Supply, administer and prescribe medicines as indicated (and appropriate) to address patient need, according to medicines management policy, Patient Group Directions (PGDs), NMC standards or referring to registered GP as necessary. Provide information and advice on prescribed or over the counter medication, medication regiments, side effects and interactions. To recognise the need for and initiate where appropriate relevant diagnostic investigations, using their skills in this area to take the correct course of action. Interpret test and investigation results as appropriate and advise patients of next steps. Understanding local safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults.
About us Sandy Health centre is located in Sandy Bedfordshire is a purpose built building looking after patients in the town and surrounding villages. We have in circa 10.100 patients. Our clinical team consists of 5 GP partners, salaried GP, physician associate, advanced nurse practitioner, nurse, HCA and phlebotomist. The clinical team is supported by a good admin team and dispensary staff. We now have a PCN pharmacist and pharmacy technician assigned to our practice to help with medication reviews. We are part of Sandhills PCN which covers two practices two in Sandy and one in Biggleswade and work closely with each other to improve patient care. We work well as a team to offer excellent medical care to our patients and to support each during the working day as we all know how busy it can get.
