Full job description

Do you want to join an innovative yet well established social prescribing team?

The available role is a Children’s Health and Wellbeing Navigator who acts within their professional boundaries to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing.

The role will be to increase patient ability to access and utilise community support whilst providing access to self-management education, peer support and social prescribing.

Health and wellbeing navigators take an approach that considers the ‘whole person’ in addressing existing issues and encourages proactive prevention of new illnesses. The approach will be to appreciate patients’ circumstances in a non-judgemental manner. This will result in the need for positive communication and negotiation skills that will support the patient's personal choices.

The successful candidate will need a driving licence and access to their own transport as they will be working across the four practices in the PCN (Ivy Court, Charing, Hamstreet and Woodchurch) as well as completing home visits and community based meetings.

Mileage incurred during working hours is claimable. Hours are negotiable but preferred at a full time 37.5 per week worked 8am-4pm with one 'late' of 10am-6pm. We also work one in five Saturdays on a rota either 9am-1pm or 1pm-5pm remotely.

Work laptop and mobile phone are provided.

The role will be an integral part of the general practice team working in each practice and across the PCN.

The following are the core responsibilities of the Children’s Health and Wellbeing Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:

a. Process and effectively signpost patients to the appropriate healthcare professional or pathway

b. Answer and direct childrens social prescribing queries

c. Support the children’s coordinator to action incoming tasks upon receipt and direct as appropriate.

d. Support the coordinator in setting up MDT meetings, completing actions and onward referrals and updating notes from MDT meetings.

e. Initiate contact with and respond to requests from patients, team members and external agencies, sharing information in accordance with GDPR regulations and in accordance with the responsibilities of your role

f. Input data into the patients’ healthcare records as necessary

g. Manage queries from clinicians as necessary in an efficient manner

h. Support the team in allocating, processing and completing tasks

i. Support the PCN team with general duties as requested

j. Maintain a clean, tidy, effective working area at all times

k. Assist the PCN manager in reasonable duties in relation to your role

l. Be responsible for your own calendar management

m. Complete allocated administration tasks

n. Suggest and implement PCN projects

Salary will be reviewed upon completion of probation.
