Job Overview
Ready Health is seeking an experienced and compassionate ADHD Assessor and Titration Specialist to join our growing multidisciplinary team.
The successful candidate will conduct comprehensive ADHD assessments for children, adolescents and adults, contribute to diagnostic decision-making, and support patients through safe and effective ADHD medication titration.
This is an in-person, clinic-based role suited to a Pharmacist Independent Prescriber or Advanced Nurse Practitioner with relevant experience in ADHD, mental health, neurodevelopmental conditions or prescribing.
The postholder will work autonomously within their professional competence while collaborating with other ADHD clinicians, prescribers, GPs, educators and healthcare professionals. The role requires excellent clinical judgement, strong communication skills and a commitment to delivering evidence-based, patient-centred care.
Key Responsibilities
ADHD Assessment
- Conduct comprehensive ADHD assessments for children, adolescents and adults using recognised diagnostic criteria, structured clinical interviews and validated assessment tools.
- Gather detailed developmental, educational, behavioural, psychological, social and medical histories from patients, parents, caregivers and other relevant informants.
- Review previous medical records, school reports, questionnaires and supporting documentation.
- Assess the impact of symptoms across different settings, including home, education, employment and relationships.
- Screen for differential diagnoses, coexisting mental health conditions, neurodevelopmental conditions and relevant physical health concerns.
- Contribute to multidisciplinary diagnostic discussions and formulate clear, evidence-based clinical conclusions.
- Prepare accurate and comprehensive assessment reports, including diagnosis, clinical formulation and recommendations.
- Provide clear and sensitive feedback to patients and families following assessment.
ADHD Medication Titration and Prescribing
- Conduct baseline clinical reviews before starting ADHD medication, including relevant medical history, current medication, cardiovascular risk, blood pressure, pulse, weight and height.
- Discuss available treatment options, expected benefits, potential adverse effects and appropriate monitoring requirements.
- Initiate and titrate ADHD medication within professional competence and in accordance with current clinical guidance, prescribing standards and Ready Health policies.
- Monitor treatment response, symptom improvement, functional outcomes, adherence, side effects and physical observations.
- Adjust medication doses or treatment plans where clinically appropriate.
- Identify patients who require additional investigations, an ECG, specialist advice or referral before treatment can proceed.
- Provide repeat prescriptions where clinically appropriate and ensure all controlled drug prescribing requirements are followed.
- Support patients until they are considered stable and optimised on treatment.
- Prepare treatment summaries and shared care requests for the patient’s GP where appropriate.
- Conduct ongoing medication reviews for patients who remain under private prescribing arrangements.
Patient Care and Communication
- Develop individualised treatment and management plans that reflect the patient’s goals, circumstances and clinical needs.
- Provide patients and families with practical guidance regarding medication, symptom management, lifestyle, education and workplace support.
- Explain complex clinical information clearly, sensitively and without unnecessary jargon.
- Support informed decision-making and obtain appropriate consent throughout assessment and treatment.
- Recognise and respond appropriately to safeguarding concerns, deterioration in mental health, medication misuse or other clinical risks.
- Communicate effectively with GPs, schools, colleges, employers and other professionals where consent has been obtained.
Clinical Governance and Record Keeping
- Maintain accurate, detailed and contemporaneous clinical records.
- Complete reports, prescriptions, correspondence and follow-up documentation within agreed timescales.
- Work in accordance with data protection legislation, professional confidentiality requirements and Ready Health policies.
- Follow relevant safeguarding, controlled drug, prescribing, incident reporting and clinical governance procedures.
- Participate in clinical audits, case reviews, peer discussions and service development activities.
- Work within professional scope of practice and seek advice or refer patients where their needs fall outside personal competence.
- Maintain appropriate professional indemnity and registration with the relevant regulatory body.
Essential Requirements
- Registered healthcare professional with current UK registration as either:
- a pharmacist registered with the General Pharmaceutical Council, or
- a nurse registered with the Nursing and Midwifery Council.
- Independent prescribing qualification.
- Relevant post-registration clinical experience.
- Experience assessing, reviewing or managing patients with ADHD, mental health conditions or neurodevelopmental needs.
- Knowledge of ADHD presentation across children, adolescents and adults.
- Experience taking comprehensive clinical, developmental and medication histories.
- Competence in interpreting physical observations and assessing medication suitability.
- Strong clinical decision-making and risk-assessment skills.
- Excellent written communication, clinical documentation and report-writing skills.
- Ability to explain diagnoses and treatment options clearly and empathetically.
- Ability to work independently while contributing effectively to a multidisciplinary team.
- Strong organisational and time-management skills.
- Commitment to safeguarding, confidentiality, equality and patient-centred care.
- Evidence of ongoing continuing professional development.
Desirable Requirements
- Formal ADHD assessment or diagnostic training.
- Experience using structured ADHD assessment tools such as DIVA-5, Young DIVA-5, SNAP-IV, ASRS or equivalent validated tools.
- Previous experience initiating and titrating stimulant and non-stimulant ADHD medication.
- Experience working within a private ADHD, mental health, paediatric or primary care service.
- Experience working with both adults and children.
- Experience preparing ADHD diagnostic reports and shared care documentation.
- Knowledge of relevant NICE guidance and professional prescribing standards.
- Training in safeguarding children and adults.
- Experience liaising with schools, SENCOs, GPs and other healthcare professionals.
- Teaching, supervision, mentoring or clinical education experience.
