JOB TITLE: Practice Nurse (Fixed term contract)
SALARY: Dependant on experience

ACCOUNTABLE TO: Practice Manager and Clinical Lead
JOB SUMMARY:
The post-holder will be a Registered Nurse with current NMC registration, working within their scope of professional practice to deliver high-quality nursing care to the registered population of The Mosslands Medical Practice.
The Practice Nurse will work collaboratively with the multidisciplinary team to provide evidence-based care, health promotion, chronic disease management, vaccination and immunisation programmes, cervical screening, wound care and other nursing interventions that support the health and wellbeing of patients.
The role will contribute to improving patient outcomes, supporting national and local quality initiatives, and delivering high standards of patient-centred care across the practice.
PRINCIPLE RESPONSIBILITIES / COMPETENCIES
1. Responsibilities for Patient Care
1.1

1.2
In accordance with the practice timetable, work plan and appointment templates, undertake a variety of nursing duties including but not limited to:
· Long-term condition reviews (within competency)
· Cervical cytology
· Childhood and adult immunisations
· Travel vaccinations (where trained)
· Venepuncture
· ECG recording
· Blood pressure monitoring
· Lifestyle advice and health promotion
· Telephone consultations where appropriate Management of clinical correspondence and results within agreed protocols.
· To conduct home visits for patients, on a rota with the nursing team ensuring timely and effective housebound treatments.
· Use critical thinking in the clinical decision-making process to make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

Assess, plan, implement and evaluate evidence-based nursing care within professional competence
Support patients to self-manage long-term conditions through education, advice and personalised care planning.
Screen patients for disease risk factors and promote healthy lifestyles.
Provide smoking cessation, weight management and lifestyle advice where appropriate.
Administer medicines and vaccinations in accordance with Patient Group Directions (PGDs), Patient Specific Directions (PSDs) and local policies.
Recognise deteriorating patients and escalate concerns appropriately to a GP or other senior clinician.
Refer patients to other members of the multidisciplinary team or external services in accordance with practice protocols.
Maintain accurate, clear and contemporaneous clinical records in accordance with NMC standards and practice policy.
Undertake telephone and digital consultations where appropriate and within professional competence.
Participate in recall systems and proactive management of patients with long-term conditions.
Support infection prevention and control procedures.
Contribute to the delivery of safe, effective and compassionate nursing services.
Use population health data to identify patients who would benefit from preventative interventions and health promotion activities.
Comply with all relevant practice policies including confidentiality, safeguarding, health and safety, infection control and information governance.
Recording clear and contemporaneous consultation notes to agreed standards, collecting data for audit purposes
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with working along side GPs within primary care
2. Responsibilities for Human Resources
All staff will be familiar with and abide by HR policies and procedures to support the smooth running of the company and will contribute to adequate clinical cover across sites.
3. Responsibilities for physical and financial resources
Support TMMP efforts to maximise practice income through schemes including QOF and Salford Standard
Support TMMP efforts to appropriately control practice and NHS expenditure e.g. cost effective prescribing, appropriate referrals, waste reduction.
4. Communication & Team work
The post-holder should recognise the importance of effective communication within the team and will strive to:

Undertake mentorship, teaching and supervision for staff of all levels including doctors, medical students, nurses, student nurses, AHPs and admin staff, assessing competencies against set standards.

The post-holder will:

- Generate and contribute to ideas for quality improvements (QI)
- Engage with TMMP culture that promotes quality improvements
- Support and engage with the implementation and embedding of positive QI changes
- Discuss with other members of the team how the policies, standards and guidelines will affect own work.
6. Partnership Working
Contribute to good team working and positive working relationships with colleagues in and beyond TMMP.
7. Analysis and Data Management
All staff will be familiar with and observe all service processes and protocols relating to accurate data
collection and contribute as necessary to discussions regarding data analysis.
8. Information Management
Contributing to the development of computer-based patient records
Contributing to the summarising of patient records and read-coding of patient data
Contribute to excellent Information Governance.
9. Clinical Governance and Performance

The post-holder will strive to maintain quality within the practice, and will:

12 Physical, Mental & Emotional Demands of the Post
This post can be mentally and emotionally demanding. The nature of this role may include dealing with vulnerable patients, patients with complex needs and/or difficult personal circumstances, exposure to extremes of emotion and psychological distress.

13.5 Engage with 1-2-1 meetings and NHS and TMMP appraisal processes.

14 Review
These guidelines are provided to assist in the performance of the contract but are not a firm condition of the contract. This job description will be reviewed as necessary to meet the needs of the service, in consultation with the post holder.
