This role is primarily based at our Head Office in Mansfield, Nottinghamshire, with occasional community-based responsibilities including staff observations and service user visits.
We are currently seeking an experienced, motivated, and professional Care Coordinator to join our management team and support the day-to-day delivery of our domiciliary care services.
The Role:
**Please note this role is a temporary 12 month contract, covering Maternity Leave **

Our Care Coordinators play a key role in ensuring safe, effective, and compassionate care delivery while supporting operational performance and staff wellbeing.
Key Responsibilities:
The successful candidate will be responsible for:
- Working effectively with colleagues and management teams in a professional and respectful manner
- Taking accountability for the performance and delivery of care within your allocated area, ensuring all visits are delivered safely, effectively, and to a high standard
- Planning, reviewing, and completing weekly rotas for all employees, focusing on logistics, efficiency, continuity of care, and staff morale
- Using data and performance insights, including travel time, on-time visits, staffing levels, and service user requirements, to improve operational efficiency and care delivery
- Analysing and interpreting data from care systems to identify trends, risks, and opportunities for continuous improvement
- Monitoring key performance indicators (KPIs) and taking ownership of outcomes within your team
- Working closely with the Registered Manager to ensure service user requests and care needs are met wherever possible
- Collaborating with other Team Managers to ensure consistency across the business and maintain high standards of care delivery
- Managing and responding proactively to live rota issues and operational challenges as they arise
- Communicating clearly and effectively with staff to support wellbeing and maintain safe care practices
- Monitoring and responding efficiently across multiple communication channels
- Managing staff performance in line with company policies and procedures
- Supporting HR processes including, probation reviews, disciplinary meetings, and staff development
- Inducting, mentoring, and supporting new team members to help build a positive, accountable, and high-performing team culture
