Job Overview
We are seeking a compassionate and dedicated Care Manager to lead and coordinate comprehensive care services for our senior residents. Duties
- Develop, implement, and regularly update personalized care plans tailored to each resident’s needs, preferences, and medical conditions
- Coordinate with healthcare professionals, families, and staff to ensure seamless delivery of medical and personal care services
- Administer medications accurately following prescribed schedules and medical terminology guidelines
- Monitor residents’ health status, behavior management needs, and safety protocols to promote optimal well-being
- Assist with activities of daily living such as meal preparation, mobility support, and light housekeeping tasks
- Conduct regular assessments to evaluate care effectiveness and adjust plans accordingly
- Lead staff training on topics including Alzheimer’s care, dementia management, hospice care, and heavy lifting safety
- Ensure compliance with all regulatory standards related to patient care, medication administration, and case management
Skills
- Proven experience in nursing home or assisted living environments with a focus on senior and dementia care
- Strong knowledge of medical terminology, medication protocols, and case management practices
- Excellent communication skills for collaborating with healthcare teams, families, and residents
- Ability to handle behavior management challenges with patience and professionalism
- Valid driver’s license with reliable transportation for home visits or community outings
- Hands-on experience in caregiving roles involving Alzheimer’s or hospice care is highly desirable
- Physical stamina for heavy lifting tasks related to resident mobility assistance or equipment setup
- Competence in developing care plans that prioritize safety, comfort, and independence

Pay: £35,270.17-£43,270.99 per year
