Position: Experienced Permanent Support Worker / Rehab Coach to complete 1:1 role to support our client in his own home
Hours: 16 hours per week, 8 hour shifts (In particular; Monday, Wednesday and Thursday)
Please note that there will be a requirement to work a combination of these shifts.
Hourly rates Day Rates weekday: £14.03 - Day Rate weekend: £14.63
Location: OL3 (Near Delph)
Position subject to enhanced DBS, 2 x references, Right to Work in the UK.

Role description
1. To support our client to develop a wide-ranging list of community interests and activities. This will be required for both wet/dry weather days to ensure that there is always an achievable option. Current options include attending the gym, pool/snooker, cinema, walks, visiting National Trust properties etc
2. The role will involve supporting the client on short breaks and holidays which will require overnight stays.
3. To enable and support the client in maintaining his home. This will involve laundry tasks, cleaning, meal planning, shopping and meal preparation. The role will also involve supporting the client with home maintenance tasks and obtaining quotes for any intended work.
4. To support the client in accessing therapy appointments each week and to follow through with any guidelines provided by therapists between appointments.
5. Support with planning weekly structure, making note of upcoming appointments etc.
6. Supporting our client to ensure that medication is ordered, picked up and administered as prescribed (prompts only)
7. Support with planning and budgeting and managing his finances.
8. To cooperate with others in enabling the client to follow their support plan to best effect. This will require excellent communication skills in dealing liaising with the Deputy, Case Manager and other professionals involved.
Person requirements
A full, clean driving licence and willingness to work in a home with pets are essential.
The ideal candidate will:
- Have experience of working with individuals with an acquired brain injury
- Possess excellent communication skills and a good sense of humour
- Be an effective motivator and problem-solver
- Be able to promote independence while balancing risk and safety
The focus of the role is to support our client to lead a fulfilling and meaningful life. Full training and regular supervision will be provided to successful candidates.
Pay: £14.03-£14.63 per hour
