TITLE : SUPPORT WORKER
SERVICE: TO SUPPORT INDIVIDUALS TO LIVE IN THE COMMUNITY, WITH PEERS OR WITHIN A FAMILY
RESPONSIBLE TO: Registered Manager, TRINITY CARE AND SUPPORT
HOURS: Flexible, to include evening and weekend work
HOURLY RATE: £13.45 (per hour)
Travel expenses will also be paid for journeys in excess of 4 miles, at a rate of 47p per mile.
MAIN AIM OF JOB:
Ø TO PROVIDE PRACTICAL SUPPORT TO PEOPLE WITH A LEARNING DIFFICULTY/DISABILITY AND THEIR CARERS
Ø TO PROMOTE EACH INDIVIDUAL’S CHOICE AND MAINTAIN INDEPENDENCE
Ø TO HELP IMPROVE QUALITY OF LIFE FOR INDIVIDUALS AND ASSIST THEM TO MAXIMISE OPPORTUNITIES FOR COMMUNITY INVOLVEMENT
RESPONSIBILITIES
Ø Support Workers are responsible to the Registered Manager (RM) for the efficient conduct of the duties they perform with individuals
Ø To maintain confidentiality about an individual’s affairs
Ø To report immediately to the Registered Manager (RM), and if appropriate the Carer, any difficulties encountered or incidents occurring during the period of support
Ø To sign and adhere to the Code of Practice laid down by T C & S
Ø To preserve dignity, independence and motivation of individuals
Ø To comply with all policies and procedures of T C & S
KEY TASKS/PRACTICAL DUTIES
· To offer general support to the individual in their own home as required, and/or to take part in leisure activities in a community setting
· To follow the routines of personal care and hygiene and to assist individuals to undertake domestic tasks in a manner which promotes their independence and acquisition of skills
· To provide a safe environment for individuals in all support settings
· To be familiar with Health and Safety at Work regulations, as applicable, and ensure their effective implementation
· To offer support in a sensitive manner, taking account of the individual’s abilities and emotional needs
· To follow individual development programmes as appropriate
· To maintain written records as necessary
· To participate in appropriate training and attend meetings as required
· To co-operate with the main carers, colleagues and other members of the Care Team in the statutory authority, to maintain a high standard of service, reporting any change in need to the Registered Manager
· To act as an appropriate role model when supporting individuals. This may include: -
1. Participating in activities as appropriate
2. Dressing appropriately for specific activities being undertaken
· Any other duties commensurate with the position, and agreed with the post holder, in the light of changing circumstances
· To deliver support/care in such a way as to meet the standards of care set by the company and to comply with T C & S’ registration with CIW
WORKING CONDITIONS/ CONDITIONS OF EMPLOYMENT
· All staff must be aware that they should be prepared to work at any location and with individuals selected by the organisation. (This selection will be based on the post holder’s training and experience)
· The pattern of work is subject to review, and reasonable adjustment will be made in accordance with the needs of the service
· All information about individuals, the company and staff is strictly confidential. Any breach of confidentiality may be subject to disciplinary action.
· The post is subject to health screening, DBS check, references, attendance at induction training and a 6 month probationary period.

· To arrange business insurance on any vehicle used for the transportation of Service Users in connection with their support package
