Overview
We are seeking a dedicated and dynamic Care Co-ordinator to join our compassionate healthcare team. In this vital role, you will oversee and coordinate care services for our residents, ensuring personalized, high-quality support that promotes independence and well-being. Duties
- Develop and implement tailored care plans for each resident, ensuring their individual needs and preferences are met with compassion and professionalism
- Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with relevant regulations
- Coordinate schedules and supervise care staff to deliver consistent, high-quality care across all shifts
- Communicate effectively with residents, families, healthcare professionals, and team members to ensure seamless care delivery
- Monitor residents’ conditions regularly, adjusting care plans as needed to promote health and safety
- Manage transportation arrangements, including driving care workers to care visit as required
- Maintain accurate documentation of care activities, incidents, and updates using IT systems to ensure compliance and continuity of care
- Lead team meetings, provide supervision, and support staff development to foster a motivated and skilled caregiving team
- Ensure proper documentations of works completed.
- Ensuring safe medication management protocols are followed and evidenced
- Work with the Manager, HR, Admin and other relevant personnel to ensure good governance, including evidencing audit trails.
Qualifications
- Proven experience in senior care or working within a care environment with strong administrative skills.
- Strong leadership skills with the ability to supervise and motivate a diverse team of care professionals
- Excellent communication skills to liaise effectively with residents, families, and healthcare providers
- Demonstrated ability to develop comprehensive care plans tailored to individual needs
- Strong organisational abilities with attention to detail, capable of managing multiple tasks simultaneously.
- Valid driving license with reliable access to a vehicle for transportation duties
- Proficiency in IT systems for documentation and reporting purposes
- Knowledge of safeguarding procedures, health & safety regulations, and compliance standards in senior care settings
- Familiarity with medical terminology is advantageous but not mandatory.

Pay: £30,000.00-£31,200.00 per year
