We are offering an exciting opportunity to join our Home First (Discharge to Assess – D2A) Service as a Shift Lead based in Tamworth.

The service supports hospital discharge to assess and re-able in the home and you will support the delivery of the service in line with service demand. Due to the nature of this service you would be required to support the service 7 days a week between 7am-10-pm on a rota basis, which includes weekends and
You will support the Integrated Team Lead in meeting the Care Quality Commission Standards and as part of this role you will:
Roster weekly staff work programmes.
Manage and supervise staff.
Provide direct hands-on care in the community as required.
Contribute and support staff with meeting and reviewing individual patient goals and outcomes based on their assessed need.
We are looking for candidates with a strong work ethic and values to provide an excellent standard of care. Excellent communication and feedback skills are required for this post.
As this is a community-based post it is essential that you are able to travel to various locations across the geographical area of the service, therefore you must be able to travel effectively and efficiently across the patch.
As Shift Lead you will be responsible for the line management of Home First Promoting Independence Workers (PIW), to deliver a timely high standard and qualitative service to individuals within their own homes including managing an off-duty rota for PIW's.
You will also, as part of this role:
Be proactive in influencing and developing the service provision
Monitor the care delivered to individuals, working closely and in partnership with health and social care professionals, voluntary and third sector
Deliver personal care to patients.
Facilitate individuals transition to alternative services & support as needed
Assist with the production of timely and accurate management information in respect of quality of services delivered.
Provide robust links between PIW’s, qualified professionals, individuals and carers within their own home
