West Berkshire Council's Adult Social Care Service is seeking dedicated Social Care Practitioners to join our team and contribute to the delivery of high-quality social care services.
The Role
As a Social Care Practitioner, you will be responsible for:

Working collaboratively with individuals to assess their social care needs and provide appropriate support and interventions.
- Conducting comprehensive assessments and reviews of individuals' care requirements, ensuring their needs are met.
- Building positive relationships with service users, their families, and other professionals to promote effective care planning and delivery.
- Providing advice, guidance, and advocacy to individuals, empowering them to make informed choices about their care.
- Ensuring compliance with relevant legislation, regulations, and organisational policies.
This is a fixed term contract 1 year from start date (Funding limited project)
The Ideal Candidate
The ideal candidate will possess the following skills and attributes:

A relevant social care qualification and registration with the appropriate professional body.
- Experience working in a social care setting, ideally within an local authority environment.
- Sound knowledge of social care legislation, regulations, and best practices.
- Excellent assessment and care planning skills, with the ability to identify and address individuals' needs.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and collaborate with multidisciplinary teams.
- Ability to work independently, manage a diverse caseload, and prioritise tasks effectively.
- A positive and solution-focused approach, demonstrating resilience in challenging situations.
