Herefordshire Mind is an existing partner in the Community Mental Health Transformation Project that aims to fundamentally remodel the way community mental health services operate. We want to break down barriers and be able to create a smooth journey for people travelling through the mental health system.
We are looking for a Link Worker to work 22.5 hours a week covering the South & West and East of Herefordshire.
Our Link Workers are not just about signposting. We sit alongside those we work with and deliver a flexible service suited to them.
Link Workers are imbedded within Neighbourhood Mental Health Teams and support individuals with practical issues that could be affecting their mental health. These issues could include housing, debt advice, social isolation and benefits. You will work to ensure individuals are linked in with the appropriate statutory and voluntary services. You will support people at all stages of their recovery journey and enable opportunities for peer support.
We are looking for someone who has experience of working positively with individuals with mental health issues and with an understanding of the impact poor mental health can have on a person’s life and the complex social and emotional challenges they may face.
Your positive approach and the ability to really connect and engage with people, will motivate people to reach their goals and enable you to develop excellent relationships with health professionals within the Neighbourhood Teams, wider NHS health teams and the voluntary sector. We are looking for excellent communicators with strong planning and organisational skills who have a flexible and positive approach to their work.
Herefordshire Mind is a workplace where you’re encouraged to be your authentic self without fear of discrimination on any grounds, with the list not being exhaustive of ethnicity, sexual orientation, disability, neurodivergence, gender, age, living and lived experience of mental health problems. We truly believe that a range of experiences creates a better workplace and a better service.
