We have an exciting opportunity for Family Support Worker to join our current Family Support Team within the Family Wellbeing and Outreach Services for 6 month Fixed Term Contract.
About the role:
An opportunity has arisen for a 6 month fixed term job share as a Family Support Worker. In this role you will organise and facilitate a range of inclusive events and groups in the hospice and community, helping family members build peer support, reduce isolation and strengthen their connection with Tŷ Hafan.
You will use participation-based practice to support meaningful engagement, particularly with mums and female carers, as well as whole family groups.
You will also provide needs-led engagement and one-to-one emotional support to family members, working under the direction of your line manager and alongside colleagues across the Family Support Team.
Key responsibilities
- Organise and facilitate events and groups in the hospice and in community settings, shaped by the need of families.
- Support equitable access for families across the Tŷ Hafan catchment area.
- Contribute to engagement strategies, including appropriate use of Tŷ Hafan-facilitated social media and other communication methods.
- Provide one-to-one emotional support and mentoring interventions as directed by the Family Support Practitioner or line manager.
- Use assessment and risk awareness skills to identify when families may need additional support.
- Maintain accurate records and contribute to activity, outcome and impact reporting.
- Work collaboratively with colleagues, volunteers and teams across the organisation.
The post holder will be required to work 15 hours per week, across three days, from 10:00am to 3:00pm on Mondays, Tuesdays, and Wednesdays.
We are looking for someone who can work flexibly when needed, including occasional weekends, to support the Charity's activities and services.
About you:
You will be personable, approachable, and able to work sensitively in a challenging environment. You will bring experience of facilitating groups, events or activities, and recent experience of providing emotional or practical support to adults, children or families. You will be confident working independently, able to maintain professional boundaries, and skilled in adapting your communication to different audiences.
You will need a Diploma in Childcare or Health and Social Care, good IT skills including Microsoft 365, and the ability to travel across the catchment area.
