JOB TITLE: CARE NAVIGATOR
REPORTS TO: RECEPTION MANAGER
ACCOUNTABLE TO: Reception and HR Manager
HOURS:
2 Positions
1 x 19 hours - Monday and Tuesday 8am-6.30pm
1 x 19 hours - Wednesday and Friday 8am-6.30pm
Job summary:
Keystone GP Partnership are recruiting for 2 care navigator positions to work across our expanding practices. The partnership has recently taken over Manston surgery to now run both practices under the Keystone GP Partnership name. Our experienced leadership team is helping to deliver modern general practice taking advantage of at scale working while also maintaining consistent patient care from teams that know patients well.
Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone
Job responsibilities:
Reception Duties
- Navigate patients to the most relevant treatment provider.
- Utilise appointment system and monitor flow of patients into consulting and treatment rooms.
- Assist patients with understanding practice operating procedures.
- Advise patients of relevant charges for private services, accept payment and record in line with agreed protocol.
- Respond to all queries and requests for assistance from patients and visitors.
- Enter requests for home visits onto the computer following practice protocols.
- Ensure prescriptions are actioned via all channels following practice protocols.
- Action requests for ambulances following practice protocols.
- Ensure reception and waiting areas are kept neat and tidy.
- Adhere to correct opening and closing procedures for the practice.
Management of appointment systems
- Book appointments according to practice protocol.
- Report any problems with appointment system to the management team
Operation of telephone system
Receive and make calls as required. Divert calls and take messages as appropriate, following the practice protocol.
Management of medical records
- Register new patients following practice protocol.
- Ensure correspondence, reports, results etc are filed in the correct record following practice protocol.
Administration/computer duties
· Undertake any delegated individual jobs as per practice protocols
· Liaise with the Workflow Co-ordinators regarding any administrative problems.
· Undertake any additional training required for newly delegated tasks.
· Any other delegated duties considered appropriate to the job.

Pay: £12.71 per hour
Ability to commute/relocate:
- Leeds LS15: reliably commute or plan to relocate before starting work (preferred)
Experience:
- Medical Receptionist: 1 year (preferred)
