About the Role
As a Care Coordinator, you will be responsible for coordinating care services and ensuring that care visits are delivered efficiently, safely and in line with client requirements.
Your duties will include:
- Planning and maintaining care staff rotas
- Coordinating and scheduling client visits
- Ensuring continuity of care wherever possible
- Liaising with clients, families, care staff and healthcare professionals
- Monitoring service delivery and responding to changes in client needs
- Supporting recruitment, onboarding and training activities
- Maintaining accurate records and electronic care systems
- Ensuring compliance with company policies and regulatory requirements
- Supporting the on-call service as required
What We Are Looking For
- Previous experience in a Care Coordinator role or similar position within Health & Social Care.
- Experience using electronic rostering or care planning systems.
- Excellent organisational and time management skills.
- Strong communication and interpersonal abilities.
- Ability to work effectively in a fast-paced environment.
- Good knowledge of domiciliary care services and client-centred care.
- NVQ Level 3 in Health & Social Care (desirable).
- Right to Work in the UK. We are currently unable to offer sponsorship.
