Summary
We are seeking a dedicated Care Co-ordinator with experience in working with those with learning disabilities to join Forest Homecare in the GB region. Based in Sible Hedingham, this vital role involves organizing and overseeing care services to ensure our clients receive the highest quality support. As a key member of our team, you will help us uphold our mission to deliver compassionate community-based care.
Responsibilities
- Coordinate and manage daily care schedules to meet individual client needs.
- Supervise and support care staff ensuring compliance with care plans and company policies.
- Develop and review personalized care plans in collaboration with clients and their families.
- Maintain accurate documentation and records of all care activities.
- Communicate effectively with clients, families, and healthcare professionals.
- Ensure adherence to health and safety regulations.
- Support staff training and development initiatives.
- Promote a positive team environment aligned with our community-focused values.
- Qualifications
- Proven experience in supervising or managing care teams.
- Strong leadership and organizational skills.
- Excellent communication abilities.
- Valid driving license is essential.
- Experience with care planning software or IT systems is advantageous.
- Knowledge of local community resources is a plus.
- Ability to work independently and as part of a team.
