The Community Therapy Team provides a high-quality, responsive rehabilitation service for adults, helping people recover following illness or injury, manage long-term conditions and improve their health and wellbeing.
Based at Tooting Works in the London Borough of Wandsworth, you will join a friendly and supportive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Rehabilitation Support Workers and Trusted Assessors, working together to deliver person-centred care.
This is an exciting opportunity to take professional responsibility for assessing, planning and delivering occupational therapy interventions for adults referred to the service. Working closely with service users, carers and partner organisations, you will support people to achieve meaningful rehabilitation goals, maximise independence and enhance their quality of life. You will also have opportunities to contribute to service development, quality improvement and service evaluation, helping to shape safe, effective and responsive community rehabilitation services.
Provide occupational therapy assessment and intervention for adults across home and community settings, promoting independence, wellbeing and participation in meaningful activities
Undertake assessments of mobility, posture and seating needs, under appropriate supervision , within patients' homes, residential and nursing care settings, and other community locations as required .
Develop, implement and review person-centred treatment plans in partnership with service users, families and carers to support the achievement of agreed rehabilitation goals.
Work collaboratively with members of the multidisciplinary team to deliver coordinated, high-quality rehabilitation and maximise service users' independence.
Establish and maintain effective working relationships with health, social care and voluntary sector partners, including General Practitioners, acute providers, District Nursing teams, Wheelchair Services and colleagues across the organisation, to facilitate timely and appropriate referrals .
Demonstrate an understanding of the occupational therapy role within integrated community services and contribute positively to multidisciplinary team working.
Contribute to service development, audit, research and quality improvement activities to support the delivery of safe, effective and responsive services.
Maintain professional competence through active participation in Continuing Professional Development (CPD), supervision, mandatory training and reflective practice.
