We’re looking for a motivated, organised, and compassionate Care Coordinator to join our collective Integrated Community Teams with partner organisations — a dynamic group of clinicians, practitioners, and community partners all dedicated to delivering outstanding, joined‑up care.
As a Care Coordinator, you’ll be at the heart of our multidisciplinary team (MDT), ensuring everyone stays connected and informed. Your role will include:
Coordinating MDT meetings and streamlining communication
Supporting proactive, personalised care planning for patients with complex needs
Working closely with GPs, nurses, therapists, social care, and voluntary sector partners
Helping the team deliver seamless, patient‑centred care across our neighbourhood
Keeping systems updated and making sure the right information flows to the right people at the right time
Your work will improve outcomes, reduce barriers, and help people stay healthier at home — where they want to be.
