About Annicare Lincoln
Annicare Lincoln is looking for an experienced and dedicated Social Worker to join our team and take responsibility for managing complex care packages for children and young people aged 14 and over.
This is an exciting opportunity for an experienced professional who is passionate about supporting individuals with learning disabilities and ensuring that care is personalised, safe, effective and focused on achieving the best possible outcomes.
The Role
You will be responsible for managing and overseeing complex care packages, working closely with children, young people, families, care teams, social care professionals and other key stakeholders.
The role will include:
Managing and coordinating complex care packages for children and young people aged 14+
Ensuring care plans are person-centred, effective and regularly reviewed
Working collaboratively with families, professionals and multidisciplinary teams
Mandatory attendance at relevant Multidisciplinary Team (MDT) meetings
Actively contributing to MDT discussions, reviews and care planning
Attending and contributing to reviews, case conferences and professional meetings as required
Supporting safeguarding and risk management processes
Ensuring individuals' rights, choices and wellbeing remain at the heart of care delivery
Contributing to assessments, reviews and best-interest decision-making
Ensuring care packages meet relevant legal, regulatory and best-practice standards
Maintaining accurate and up-to-date records and documentation
Essential Requirements
A recognised Social Work qualification and current Social Work registration
Minimum of 5 years’ experience working with people with learning disabilities
Proven experience managing complex care packages
Best Interest Assessor (BIA) qualification – essential
Strong knowledge of safeguarding, the Mental Capacity Act and best-interest decision-making
Experience working within multidisciplinary teams
Excellent communication, organisational and assessment skills
A genuine commitment to improving outcomes for children and young people
Ability and willingness to attend mandatory MDT meetings and other professional meetings
