Job Description:
Cromwell Medical Staffing is currently seeking an experienced Band 5 District Community Nurse to join our client, Sirona Care & Health. This is an excellent opportunity for a skilled community nurse to deliver high-quality patient care within the community setting, supporting patients in their own homes and promoting independence.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualised care plans
- Deliver nursing interventions including medication administration, wound care, and continence management
- Support patients with self-management of long-term conditions
- Work collaboratively with GPs, therapists, social workers, and multidisciplinary teams
- Manage and prioritise a community caseload effectively
- Maintain accurate and timely patient documentation
- Participate in clinical governance, audits, and quality improvement initiatives
- Promote evidence-based practice and high standards of patient care
Requirements:
- Valid NMC Registration
- Community Nursing / District Nursing experience required
- Competent in patient assessment and care planning
- Experience managing a community caseload
- Minimum 6 months NHS experience (within the last 2 years)
- Enhanced DBS (physical copy required)
- Up-to-date Mandatory Training
