We are seeking to recruit a ‘Carer Support Practitioner – Adult Carers’ in our Swindon team.

Job Purpose
To provide personalised, holistic and strengths‑based support to adult carers across Swindon. This includes completing statutory Carers Needs Assessments, developing support and contingency plans, offering information and advice, managing Emergency Card Scheme plans, and providing ongoing casework and wellbeing support.
Practitioners work collaboratively with Swindon Borough Council (SBC), health, education and VCSE partners to improve outcomes for carers, ensuring contract requirements are met and best practice under the Care Act 2014 is consistently applied. All practitioners work flexibly as part of a multi‑skilled team, with no hand‑offs unless required for risk or statutory reasons.
