Location: HLMG Lindum Business Park
Hours: Part-time (8 hours per week)
The Social Prescriber will support individuals to improve their health and wellbeing by connecting them to non-medical community services. Working as part of a multidisciplinary team within primary care networks, they help people address social, emotional, and practical needs that impact their health, such as loneliness, housing, employment, or financial challenges.
Involved primary responsibilities:
Working closely with the patient Social Prescriber will establish and provide:
· Provide person-centred support through one-to-one consultations
· Assess individuals’ needs, strengths, and goals using a holistic approach
· Develop personalised care and support plans
· Link people to appropriate community, voluntary, and statutory services
· Build strong relationships with local organisations and community groups
· Support individuals to manage long-term conditions and improve wellbeing
· Empower clients to build confidence, resilience, and independence
· Maintain accurate, confidential records of interactions and outcomes
· Work collaboratively with GPs, nurses, and other healthcare professionals
· Safeguard vulnerable adults and follow relevant policies and procedures
What we are looking for:
- GCSE (A* - C) or equivalent in Maths and English
- Excellent communication skills (written and oral)
- Clear, polite telephone manner
- Effective time management (planning and organising)
- Problem-solving and analytical skills
- Problem solver with the ability to process information accurately and effectively, interpreting data as required
- Sensitive and empathetic in distressing situations
- Ability to work under pressure/in stressful situations
- Able to communicate effectively and understand the needs of the patient
- Experience of working in a primary care environment (desirable)
The post holder will be enrolled in (or be prepared to enrol in and complete the next available course), undertaking or qualified from appropriate training as set out by the Personalised Care Institute
The post holder will need to work flexibly to meet the needs of individuals within the primary care network and this may include working in our Extended Hours service.
The post holder will need to hold a full driving licence and be prepared to travel across the PCN area.
