This role helps people in the community by supporting rehabilitation and daily living, making a real difference to their independence and wellbeing.
You’ll work as part of a friendly multi-disciplinary team, including occupational therapists and physiotherapists. Your work will help patients recover, stay active and manage long-term conditions. You’ll use your skills to deliver care that is safe, person-centred and based on clear plans.
What you’ll do:
Support assessments and deliver agreed care and therapy plans
Monitor patients’ progress and report any changes promptly
Encourage and motivate people to take part in their care and activities
Liaise with colleagues, carers and external services to coordinate support
Maintain accurate records in line with clinical policies
What we’re looking for:
You might have a Level 4 qualification in health and social care or similar experience
Experience working in a care or rehabilitation setting
Good communication skills, including with people who have complex needs
Ability to work independently while knowing when to seek advice
A flexible, team-focused approach and commitment to high standards of care
