SALARY:
The starting salary is £24,969 per annum for working full-time 37 hours per week.
This increases annually where incremental points are awarded until the top of the salary scale is reached.

ABOUT THE ROLE
We are looking for a Carer Link Worker to join our team.
This is an exciting and rewarding opportunity to:
- Be the first point of contact for unpaid carers across Dorset
- Provide information, advice and guidance to support carers’ wellbeing and independence
- Deliver person-centred, non-clinical support through social prescribing and care navigation
- Work collaboratively with health, social care and voluntary sector partners
- You will support carers to access the right services, build resilience, and maintain their own health while caring for others.
KEY RESPONSIBILITIES
- Provide information, advice and support primarily via telephone, as well as by email and through face-to-face interactions
- Signpost and refer carers to appropriate local and national services
- Deliver one-to-one and group-based person-centred support
- Co-develop personalised support plans and track outcomes
- Facilitate GP Carer Clinics and attend community events
- Maintain accurate records using systems such as CRM and carer registers
- Promote awareness of carers’ needs within community and professional networks
- Build strong partnerships across statutory, voluntary and community organisations
- Identify gaps in services and contribute to service development
ABOUT YOU
- We are looking for someone who is:
- A strong communicator with excellent interpersonal skills
- Passionate about supporting carers and improving wellbeing outcomes
- Experienced in person-centred support or community-based roles
- Confident in managing sensitive conversations
- Organised and able to manage a varied workload
- Able to build relationships and work collaboratively across services
Desirable:
- Knowledge of social prescribing or care navigation
- Experience working with carers or vulnerable groups
- Understanding of health literacy and community-based support
ADDITIONAL REQUIREMENTS
- Ability to travel across Dorset as required
- Flexible approach to working hours, including occasional evenings/weekends
- Homeworking setup with suitable broadband
- Willingness to undertake an Enhanced DBS check
