An opportunity has arisen for a passionate and resilient Complex Lives Navigator to support individuals experiencing rough sleeping and multiple disadvantages across Weymouth.
This role focuses on engaging people who may be disconnected from traditional services, helping them access support, improve their wellbeing, and move towards greater stability and independence. Working as part of an established Rough Sleeper Outreach Team, the successful candidate will provide specialist support to individuals affected by substance misuse, mental health challenges, homelessness, domestic abuse, and other complex needs.

The Role
As a Complex Lives Navigator, responsibilities will include:
- Delivering proactive outreach support to individuals experiencing rough sleeping
- Building trusting, professional relationships using a trauma-informed approach
- Providing specialist support around substance misuse and recovery pathways
- Developing and delivering person-centred support plans
- Identifying and responding to issues relating to mental health, domestic abuse, personality disorders, and other complex needs
- Supporting individuals to access housing, healthcare, treatment services, and community resources
- Working collaboratively with partner agencies and multidisciplinary teams
- Maintaining accurate and up-to-date case records and support plans
- Participating in outreach activities, including early mornings, evenings, weekends, and on-call duties
Requirements
- Experience supporting vulnerable adults with complex needs and/or challenging behaviours
- Knowledge of homelessness, substance misuse, mental health, or related support services
- Ability to build positive working relationships with individuals who may be reluctant to engage with services
- Strong communication, organisational, and record-keeping skills
- Full UK driving licence
- Access to a vehicle for work purposes (business insurance required)

Additional Information
- Participation in an outreach rota including early mornings, evenings, and weekends
